Peter H. Vlasses, former executive director of the Accreditation Council for Pharmacy Education (ACPE), died Aug. 1, 2026, at age 78 years. Vlasses, who led ACPE for 20 years, oversaw changes to pharmacy program accreditation at a time when the profession was transitioning to the PharmD as the entry-level degree for pharmacists.

ASHP and APhA are deeply saddened by the reported harm experienced by Glenda Dorton and by the devastating impact this event has had on her family and loved ones. We also recognize that other patients were reportedly harmed, and our thoughts are with all those affected by this tragic event.

Mary Jo Reilly, who joined ASHP's drug information staff in 1964 and retired more than three decades later as the organization's chief operating officer, died June 29, 2026. She was 88 years old. At the height of her career, Reilly had administrative responsibility for ASHP’s activities in publishing, education, practice advancement, marketing, information technology, accreditation, and human resources.
