Comparison of pudendal and caudal nerve blocks for transrectal prostate biopsy in patients with anorectal disease
INTRODUCTION: Transrectal ultrasound (TRUS)-guided prostate biopsy is often associated with significant discomfort, particularly in patients with underlying anorectal conditions. Conventional local anesthesia techniques, including periprostatic nerve block — widely regarded as the gold standard — may be insufficient for adequate pain control in this patient population, thereby limiting the feasibility and tolerability of the procedure.

Optimizing recovery: An opioid-free pathway for reconstructive urology
Abstract INTRODUCTION: Even small quantities of prescribed opioids for acute postoperative pain can lead to addiction, hinder recovery, and be unnecessary. This study evaluated whether an opioid-free postoperative pathway following reconstructive urologic surgery increased pain-related patient communication. METHODS: An opioid-free postoperative protocol was implemented at our institution in 2019.
Adoption of thulium fiber laser for lithotripsy
INTRODUCTION: While holmium:YAG for laser lithotripsy remains the gold standard, the thulium fibre laser (TFL) has gained traction as a tool in urolithiasis management. The objective of this study was to review its adoption and use as a clinically effective laser lithotrite. METHODS: We performed a scoping review for articles on TFL for lithotripsy according to Preferred Reporting Items for Systematic reviews and Meta-Analyses Extension for Scoping Reviews (PRISMA-ScR).
Rates of incidental prostate cancer following HoLEP
INTRODUCTION: This study aimed to identify preoperative predictors of incidental prostate cancer (iPCa) in patients undergoing holmium laser enucleation of the prostate (HoLEP) for benign prostatic hyperplasia (BPH) and to evaluate subsequent followup strategies. METHODS: We retrospectively analyzed 571 patients who underwent HoLEP by a single surgeon between 2020 and 2024 and who attended at least one postoperative followup visit at our tertiary center.
