WHAT IS ALREADY KNOWN ON THIS TOPIC Previous research has questioned the validity of informed consent for neonatal trials where fear, anxiety and time pressure may compromise informed decision-making. Alternatives to informed consent include an ‘opt-out’ approach which may address such concerns. There is a paucity of data describing how a verbal opt-out approach is experienced by parents and operationalised by staff in neonatal trials.

Multiple, evolving cutaneous nodules present at birth in a well, term infant
A term female infant was born weighing 3.4 kg after uneventful antenatal care. There was no significant family medical history of note. Evident at birth were a series of erythematous nodular lesions on the back (10 mm by 10 mm) and groin (8 mm by 8 mm). The nodules were firm and well demarcated, with purple discolouration. Congenital diaphragmatic hernia and a hernia sac
At 21 weeks of gestation, a fetus was diagnosed with a left-sided congenital diaphragmatic hernia (CDH). At 26 weeks of gestation, fetoscopic endoluminal tracheal occlusion (FETO)1 was undertaken because of a low lung-to-head ratio (LHR) of 25%. Preterm labour occurred and the infant delivered at 27 weeks of gestation with a birth weight of 1000 g and LHR of 32%. At birth, the FETO balloon was deflated ex-utero and the baby was intubated and ventilated.

Volume of umbilical cord blood collection in the era of delayed cord clamping: a multicentre, prospective, feasibility study
Objectives To determine the percentage of adequate umbilical cord blood (UCB) collections defined as ≥70 mL of UCB after delayed cord clamping for 3 min was applied. Second, to correlate the UCB volume to gestational age at birth, birth weight and sex. Design We conducted a multicentre, prospective, feasibility study in near-term infants delivered through caesarean section between November 2023 and December 2024. UCB was collected ex-utero, immediately after the placenta was removed from the womb.
