Adenoid Cystic Carcinoma of the Perineum Presenting as a Decade of Chronic Pelvic Pain
A 59-year-old male with L4-5 spinal stenosis presented with 9 years of progressive, sharp perineal pain without inciting event, worsened with Valsalva. Pain prevented him from sitting and limited daily activities; it was worsened with erections. There was no bladder or bowel incontinence, gross hematuria, or urinary or neurologic symptoms. His previous negative workup included pelvic CT, pelvic MRI (2 years prior), perineal ultrasound, PSA, and urinalysis.
