Earlier this spring, the government announced it was moving forward with a province-wide Primary Care Medical Record (PCMR) system.
Understanding medical records rights
Our Legal Department is often asked to comment on questions related to medical records custody and portability, where a physician has left a practice and is unable to take patient medical records with them. What do members need to know?
Relocating your practice
The College of Physicians and Surgeons of Ontario Closing a practice patient notification guidelines also apply for relocation of your practice. You must provide your patients with at least 90 days' notice by letter, email, phone call or in person. When considering which channel is best for you, consider the balance of time (visits/calls) and cost (postal mail). If possible, delegate notification to your office staff.

One of your goals should be related to patient care/your work, and the other can be related to your own wellness and self-care • For each goal, describe the baseline (current state), the planned intervention(s), and the outcome measures (future state) You can refer to your Practice Profile, Self-Guided Chart Review and Data-Driven Quality Improvement activities to help you identify gaps in your practice and areas for improvement related to patient care.
Masking and PPE
If a patient refuses to wear a mask, assess what actions are appropriate in the circumstances. In general, it is inappropriate to deny necessary, urgent in-person care solely because a patient refuses to wear a mask.
