Istaroxime was shown in a small study to increase systolic blood pressure (SBP) in patients with pre-cardiogenic shock (CS) due to acute heart failure (AHF). In the current analysis we describe the effects of two doses of istaroxime 1.0 (Ista-1) and 1.5 µg/kg/min (Ista-1.5). The target dose of istaroxime, administered in a double-blind placebo-controlled fashion, was 1.5 µg/kg/min in the first cohort (n=24) and was reduced to 1.0 µg/kg/min in subsequent patients (n =36).
Constructive Criticism in Medicine: Heart Function Clinicians Leading Positive Change
Clinical rotations on the Heart Failure/LVAD/Heart Transplant services can be busy. They often demand our total immersion – managing complex hemodynamic perturbations, coordinating extracardiac care with consultants, getting to know the intricacies of patients’ personalities and support systems. Clinician fulfillment and heartache are often commingled in any given day with the rejoice of lifesaving triumphs and lament of those lost or who could not be helped.
High Output Heart Failure, Pulmonary Hypertension and Right Ventricular Failure in Patients with Arteriovenous Fistulas: A call to action
Cardiovascular disease is one of the major causes of morbidity and mortality among patients with end stage renal disease (ESRD). To this point, much of the literature has focused on the incidence and prognosis of atherosclerotic coronary disease and heart failure with reduced ejection fraction. More recently, the role of arteriovenous fistula (AVF) as a cause of high output heart failure (HOHF), pulmonary hypertension (PH) and right ventricular (RV) failure has been reported.
Time to Quadruple Guideline-Directed Medical Therapy as a Key Performance Measure for Heart Failure
The prevalence of heart failure (HF) continues to steadily rise and now more than 64 million people globally carry the diagnosis. 1 Global, regional, and national incidence, prevalence, and years lived with disability for 354 diseases and injuries for 195 countries and territories, 1990-2017: a systematic analysis for the Global Burden of Disease Study 2017.
Missense Genetic Variation of ICAM1 and Incident Heart Failure
Intercellular adhesion molecule-1 (ICAM-1) is a cell surface protein that participates in endothelial activation and is hypothesized to play a central role in heart failure (HF). We evaluated associations of ICAM1 missense genetic variants with circulating ICAM-1 levels and with incident HF.

Lifting and Healing as We Climb: Women's Heart Month
Once again this February, Heart Month prompts us to reflect on the core mission of promoting cardiovascular (CV) health, with special focus on the importance of heart health in women, who often are underdiagnosed, late to present, and less likely to receive appropriate treatment. The American Heart Association's Go Red for Women campaign is focused on this mission and carries out activities centered around the essential objectives of improving education, empowerment, and the eradication of...
Rationale, Design and Baseline Characteristics of the PARAGLIDE-HF Trial: Sacubitril/valsartan versus Valsartan in HFmrEF and HFpEF with a Worsening Heart Failure Event
Background PARAGON-HF studied the effect of sacubitril/valsartan (Sac/Val) compared with valsartan (Val) on clinical outcomes in patients with chronic heart failure with preserved ejection fraction (HFpEF) or mildly reduced EF (HFmrEF). Further data are needed regarding the use of Sac/Val in these EF groups with a recent worsening heart failure (WHF) event and in key populations not broadly represented in PARAGON-HF, including de novo HF, severely obese, and Black patients.
