January 28, 1986. Launch Complex 39-B, Kennedy Space Center. Before the eyes of a nation, Space Shuttle orbiter Challenger disintegrated 73-seconds into launch. The world watched the shocked and incredulously etched faces of family members as they grappled with reality.
As with the assignation of John F. Kennedy, most remember exactly where they were as they watched, or received the news of, this devastating tragedy. The human loss is priceless and immeasurable; and the monetary loss - unnecessary. It all could have been avoided. The entire mission fell prey to the psychological phenomena call, "group-think."
