A comparison of intramuscular (Zimhi) and intranasal naloxone (Narcan) in reversal of fentanyl-induced apnea: a randomized, crossover, open-label trial | Nature Communications
Abstract Severe opioid-induced respiratory depression (OIRD) can be treated with intranasal (IN) or intramuscular (IM) naloxone. It is relevant to compare their efficacy and determine the optimal strategy to restore breathing following OIRD.
Out-of-hospital opioid prescriptions after knee and hip arthroplasty: prescribers and the first prescribed opioid
Abstract Background We determined the first prescribed opioid and the prescribers of opioids after knee and hip arthroplasty (KA/HA) between 2013 and 2018 in the Netherlands. We also evaluated whether the first prescribed opioid dose was associated with the total dispensed dose and long-term opioid use in the first postoperative year. Methods The Dutch Foundation for Pharmaceutical Statistics was linked to the Dutch Arthroplasty Register.
Buprenorphine: a treatment and cause of opioid-induced respiratory depression
Over the last decade, physicians and the public have become aware of the opioid epidemic with thousands of deaths each year, which is today considered the second major medical disaster after the COVID-19 pandemic in the USA. In 2020, around 92 000 citizens died from an opioid overdose in the USA alone. 2 Mattson C.L. Tanz L.J. Quinn K. Kariisa M. Patel N.L. Trends and geographic patterns in drug and synthetic opioid overdose deaths—United States, 2013–2019. MMWR Morb Mortal Wkly Rep.
Overdoses due to fentanyl and its analogues (F/FAs) push naloxone to the limit
Abstract What is known and Objective Food and Drug Administration (FDA) risk evaluation and mitigation strategies (REMs) encourage emergency responders, paramedics, law enforcement agents, and even laypeople to be trained in the administration of naloxone with the intent of rescuing individuals from a known or suspected opioid overdose.
Are opioid receptor antagonists adequate for “Opioid” overdose in a changing reality?
Abstract What is known and Objective Deaths due to opioid‐induced respiratory depression (OIRD) continue to rise despite intense regulatory and professional actions. COVID‐19 has only worsened this situation.1 An opioid receptor antagonist (ORA) such as naloxone is the most common intervention for OIRD. However, with increasing overdose from highly potent illicit opioids and polysubstance abuse, appraisal of the adequacy of ORA seems warranted and timely.
Calculating positive and negative predictive values. Comment on Br J Anaesth 2021; 126: 564-7
Editor—We read the editorial by Hadjipavlou and colleagues entitled ‘What is the true worth of a p-value? Time for a change’ with great interest. We share the concern of the authors regarding the use of statistics in biomedical science. In their paper, the authors show the worth of a study's conclusion by calculating positive and negative predictive values (PPV and NPV, respectively) using equations given in their Table 1.
The conundrum of polysubstance overdose
Abstract What is known and objective Treating an opioid overdose using an opioid receptor antagonist (such as naloxone) makes mechanistic sense and can be effective. Unfortunately, the majority of current drug overdose deaths involve polysubstance use (i.e., an opioid plus a non‐opioid). Comment Respiratory depression induced by opioids results from excessive opioid molecules binding to opioid receptors. This effect can be reversed by an opioid receptor antagonist.
Ketamine for neuropathic pain: a tiger that won't bite?
Keywords efficacy ketamine neuropathic pain pain mechanisms phenotype randomised controlled trials 1 Hyperalgesia, allodynia, or both are signs of central sensitisation, a state in which there is an exaggerated response of nociceptive neurones to normal or subthreshold stimuli – that is a gain in function in relation to nociceptive stimuli. 2 Arendt-Nielsen L. From breathtaking to encapsulation: a novel approach to reverse respiratory depression from opioid overdosing
The observation that opioids reduce the chemical drive to breathe is well documented in the scientific literature. The reduced ventilatory drive is related to the slowing and eventually silencing of respiratory networks in the brainstem after opioid administration. The influence of drugs in general and opioids in particular on the ventilatory control system has been an understudied topic for a long time.

Optimizing the glutamatergic challenge model for psychosis, using S(+)-ketamine to induce psychomimetic symptoms in healthy volunteers
The psychomimetic effects that occur after acute administration of ketamine can constitute a model of psychosis and antipsychotic drug action. However, the optimal dose/concentration has not been established and there is a large variety in outcome measures. In this study, 36 healthy volunteers (21 males and 15 females) received infusions of S(+)-ketamine or placebo to achieve pseudo-steady state concentrations of 180 and 360 ng/mL during two hours.
