Arpan Nalin Patel

Arpan Nalin Patel, D.M.D., is the owner of Alpha Dental Excellence, a multispecialty dental practice at 240 Middletown Boulevard in Langhorne, Pennsylvania, serving patients throughout Bucks County and the greater Philadelphia region. A dentist in practice since 2004, Patel concentrates much of his clinical work on complex restorative dentistry, including full arch implant supported reconstruction and sedation dentistry for adults with significant dental anxiety or long delayed treatment needs.

Alpha Dental Excellence runs 23 chairs at a single address and provides general and cosmetic dentistry, dental implants, root canal therapy, oral extractions, veneers, bridges, and periodontal treatment. Housing those disciplines in one building is a deliberate choice. A patient whose treatment plan crosses several specialties can have most of it completed in the same office, with the doctors involved comparing notes directly rather than trading referral letters between practices.

Full arch reconstruction is the work Patel is best known for. An implant supported zirconia bridge spanning an entire arch requires implant placement, restorative planning, and laboratory work to line up precisely, often across months. The measure of success in those cases is function. Patients arrive unable to chew comfortably or speak clearly, and the purpose of the reconstruction is to give both back, not simply to improve the appearance of a smile.

Sedation dentistry is the other area the practice is known for, particularly among adults who have avoided a dentist for ten or twenty years after a bad experience. Arpan Patel has written about sedation as a question of psychology rather than pharmacology. Medication makes a single appointment possible. The larger objective is to rebuild enough confidence in dental care that the next visit requires less of it, and the one after that less again.

The same reasoning governs how new patients are introduced to the practice. Patel trains the doctors at Alpha Dental Excellence to follow what he calls the four minute rule: spend the first four minutes of an appointment learning about the person in the chair before raising the subject of their teeth. He learned the principle from his father, Dr. Nalin Patel, D.M.D., who taught him that patients do not want to be in a dental office and that pretending otherwise misses the point of the job.

Dentistry ran in the family well before that lesson. Nalin Patel earned his own degree at the University of Pennsylvania School of Dental Medicine and went on to build one of the larger dental groups in the region, owning roughly 26 facilities by the mid to late 1990s. His son is the second generation of that practice.

The multispecialty model came out of what Patel learned managing large practices before he owned one. From 2004 to 2011 he served as Dental Director of Advanced Dental Care in Fairless Hills, Pennsylvania, a 24 chair, multi provider, multi specialty practice staffed by more than 25 personnel. The job was half clinical and half operational. He built internal referral pathways so patients moved between general dentists and in house specialists instead of being sent outside, allocated chairs and operatories against specialist availability and case complexity, recruited and trained associate dentists, hygienists, assistants, and treatment coordinators, and set clinical and productivity benchmarks for the clinical team. He also tracked production and collections by department rather than practice wide, which is how a building that size stays healthy across specialties that bill very differently. He grew the patient base and revenue at Advanced Dental Care and at Oxford Valley Dental Excellence, a 17 chair practice, and eventually bought both.

Alpha Dental Excellence has operated in Langhorne since 2005, with Patel in a director's role from the start. He became its owner in 2011. A year later his father died unexpectedly, and the practice passed to him with two young sons at home.

What he did with it ran against the direction most of the industry was moving. Rather than expanding into a network of satellite offices, Patel concentrated the specialties, doctors, staff, and technology in one Langhorne location and built it into a large, state of the art facility with multiple doctors and dental specialties under a single roof. Patients now drive in from outside Bucks County for treatment they could not have completed in one office closer to home.

Arpan Patel studied biological sciences at the University of the Pacific in Stockton, California, graduating in May 2000, then earned his Doctor of Dental Medicine (D.M.D.) at the University of Pennsylvania School of Dental Medicine in Philadelphia in 2004. He has practiced in lower Bucks County ever since. He holds certification from the Las Vegas Institute of Aesthetic Dentistry (LVI) and implant certification from the London Institute of Implantology and from Renew Smiles. He is a member of the American Dental Association and the Pennsylvania Dental Association.

Readers voted Alpha Dental Excellence Best Dental Office in Bucks County and Patel Best Dentist in Bucks County for eight consecutive years, 2017 through 2024, in the Bucks County Courier Times Best of the Best Awards. The 2020 round brought seven categories at once, spanning the practice, its leadership, and its workplace culture.

Bill Donahue profiled Patel in Suburban Life Magazine in February 2021, in an article about the practice's first year of the COVID-19 pandemic. Alpha Dental Excellence kept treating urgent cases while many offices across Bucks, Montgomery, and Philadelphia counties were closed or running limited hours. Patients drove four or five hours from New Jersey and western Pennsylvania to be seen. He gave the credit to his management team and staff.

Leadership and clinical care are not separate categories in the way he runs the practice. He asks his doctors to start with a patient's general health and the condition of the mouth before cosmetic questions come up, which is not the order most consultations follow. With someone who has been away from a dentist for years, rebuilding trust has to come before a treatment plan does.

Outside the office he follows the Philadelphia Eagles, golfs with his two sons, and plays pickleball weekly. Arpan Nalin Patel still practices on Middletown Boulevard in Langhorne, where he takes on full arch reconstruction cases and teaches the four minute rule to every doctor who joins Alpha Dental Excellence.
