Abstract 1 Introduction Moyamoya disease (MMD) is an idiopathic progressive steno-occlusive arteriopathy involving the terminal intracranial internal carotid arteries and their proximal branches. Progressive arterial narrowing is accompanied by the development of fragile basal and periventricular collateral networks that produce the characteristic angiographic appearance described as a “puff of smoke” (1–3).
