Slow-SPEED: protocol for three randomised trials of remotely delivered exercise to prevent Parkinson’s disease
Abstract We describe the design of the first non-pharmacological Parkinson’s disease prevention trials worldwide: the randomised ‘Slow-SPEED’ trials. Three trials examine the feasibility and preliminary efficacy of a gamified, remotely administered exercise intervention vs. active control over 18–36 months in complementary prodromal subgroups: iRBD(n = 110; Netherlands), hyposmia(n = 110; United Kingdom) or LRRK2/GBA1 mutation carriers(n = 600; United States).

Abstract : The pathogenesis of Parkinson’s disease (PD) is complex, multilayered, and not fully understood, resulting in a lack of effective disease-modifying treatments for this prevalent neurodegenerative condition. Symptoms of PD are heterogenous, including motor impairment as well as non-motor symptoms such as depression, cognitive impairment, and circadian disruption.

The Covid-19 epidemic that has claimed millions of lives began in the city of Wuhan, a modern city in China with an institute (the WIV) where bat coronaviruses were studied and housed. The most basic way to determine if the SARS-CoV-2 that claimed so many victims came from the WIV would have been to find out what work was being done in this institute by obtaining funding requests and lab notebooks. This is a forensic investigation rather than a scientific study.
No surprises
Dear Editor Steven Narod finds that several of the outcomes of the Mumbai trial are surprising. Careful reading of the scientific literature indicates that they are not. « 1) The 20-year cumulative mortality from breast cancer was about the same in women under age 50 and over age 50, a far different situation from what see in Canada where older women are more likely to die of breast cancer.

Dear Editor The study of Mittra et al. (1) should be considered with attention by both supporters and opponents of mammography screening (2). In women over age 50 and older, clinical breast examination is as effective as mammography screening on breast cancer mortality and yet there is no overdiagnosis. Therefore these results confirm that the excess cancers observed with mammography screening are not overdiagnosis, but X-ray induced cancers (3,4).

Introduction Despite a strong rationale and promising beginnings, the usefulness of mammography screening is now challenged due to poor efficacy in reducing breast cancer (BC) mortality rates [1, 2]. Lack of beneficial effects of mammography screening in the long term contrasts with a better efficacy in short term clinical trials [3–5], where a 31% reduction in mortality from BC has been reported [3].
