Personalized mechanical ventilation guided by lung ultrasound in patients with ARDS: a pilot phase of a randomized clinical trial - Intensive Care Medicine Experimental
Abstract The “Personalized Mechanical Ventilation Guided by Lung UltraSound in Patients with Acute Respiratory Distress Syndrome” (PEGASUS) study aims to evaluate personalized mechanical ventilation (MV) in patients with acute respiratory distress syndrome (ARDS) compared to the standard of care. However, misclassification and misaligned MV strategies were shown to be harmful.
Site-specific complications of central venous catheterization under systematic ultrasound guidance: a target trial emulation revisiting the 3SITES study | Critical Care
Abstract Central venous catheterization is the most common invasive procedure in intensive care units but remains burdened by infectious, thrombotic, and mechanical complications. Although real-time ultrasound guidance is now widely adopted, its effect on site-specific differences in overall complication rates has not been established. The 3SITES randomized clinical trial previously demonstrated lower infection and thrombosis rates with subclavian access but higher mechanical complications.
Correction to: Anticoagulation and thromboembolic risk in critically ill patients with trigger-induced atrial fibrillation-A systematic review and meta-analysis | Netherlands Heart Journal
Correction to: Neth Heart J 2025 The version of Fig. 3 published in the original article was incorrect and has now been replaced by an amended version. The caption and any mentions of Fig. 3 within the text have also been corrected to reflect this change. The original article has been corrected. Forest plot showing the short-term a thromboembolism, b thromboembolism, c bleeding, d bleeding outcomes between anticoagulation therapy and control and e mortality and long-term.
Surrogate Endpoints in Pandemic Preparedness
To the Editor—To swiftly evaluate new therapies for a potential severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) variant, reliable surrogate endpoints are essential. Based on an analysis of trial-level data, we previously found that nasopharyngeal viral load reduction reliably predicts a reduction in the risk of hospitalization or death across trials testing new SARS-CoV-2 therapies [1]. In a recent study, Giganti et al evaluated this relationship at the individual patient level.
Effect of lung ultrasound-guided fluid deresuscitation on duration of ventilation in intensive care unit patients (CONFIDENCE): protocol for a multicentre randomised controlled trial | Trials | Full Text
Objectives and design The primary aim of this RCT is to evaluate if LUS-guided deresuscitation leads to a shorter period of invasive ventilation and/or a reduction in mortality. We hypothesise that daily LUS leads to timely detection of pulmonary oedema which triggers removal of excess fluid, leading to a more appropriate fluid status which reduces duration of invasive ventilation and might also reduce mortality.
A structured diagnostic algorithm for patients with ARDS - Critical Care
If ARDS persists for more than 5 days after diagnosis it is considered as ‘non-resolving’ and the risk of fibrosis formation is considerable if the cause for ARDS remains untreated [3, 30]. There are several studies that show that reactivation of HSV and CMV is frequent in non-resolving ARDS [31,32,33]. No randomized trials have been conducted on whether or not to treat HSV reactivations, but observational studies suggest independent excess mortality in the HSV and CMV group.
Oxygenation thresholds for invasive ventilation in hypoxemic respiratory failure: a target trial emulation in two cohorts - Critical Care
Study design, setting, and oversight This retrospective cohort study was structured as a target trial emulation (Additional file 1: Table e1). The study incorporated two deidentified patient-level databases of intensive care unit admissions: Medical Information Mart for Intensive Care IV (MIMIC-IV) [27, 29] and the Amsterdam University Medical Centers database (AmsterdamUMCdb) [30, 31].

Co-infection and ICU-acquired infection in COIVD-19 ICU patients: a secondary analysis of the UNITE-COVID data set - Critical Care
This study is a sub-analysis of the ESICM UNITE-COVID study, which has been extensively described elsewhere [1]. Briefly, UNITE-COVID is a multicenter, international, anonymized observational study carried out in 280 hospitals from 46 countries during ‘wave one’ (defined as 15th of February until 15th of May 2020).
