Cardio-Immunology: Reframing the Pathogenesis of Cardiovascular Disease
For decades, cardiovascular disease was understood primarily through metabolic, hemodynamic, and structural models that linked hypercholesterolemia and hypertension to vascular disease, atherosclerotic plaque formation, ischemic injury, cardiac hypertrophy, and ultimately to the development of heart failure.
What E.B. Meant to Me
Dr Eugene Braunwald (or “E.B.” as many of the editors of Heart Disease fondly referred to him) is the reason that I became a cardiologist. When I was a medical resident, I was considering subspecializing in nephrology or leaving medicine entirely and studying to become a neurologist. However, after reading the first edition of Braunwald's Heart Disease,1 I knew that I wanted to be a cardiologist and only a cardiologist. Even before I met Dr Braunwald, he changed the course of my life.

JACC: Basic to Translational Science 2024 Thomas Force Young Investigator Award Winner:
Introduction This is the fifth consecutive year for the Thomas Force award, which recognizes a young investigator who is the primary author for a paper published in JACC: Basic to Translational Science. The award is intended to honor not only the young investigator, but also the program in which he or she is working, in keeping with Tom’s love of mentoring. This year, we received an impressive group of nominees from exceptional mentors.
Derisking Phase II Clinical Trials in Heart Failure: A Position Paper from JACC: Basic to Translational Science and the Heart Failure Collaboratory
Introduction Phase II clinical trials play an important role in drug development by generating information that informs decision making for a promising drug candidate. The objectives of phase II development are context-dependent but can be broadly categorized into several different areas. A primary goal of phase II trials is to establish proof of concept (PoC), by demonstrating that the therapeutic agent exhibits evidence of its intended biological activity in the target population.
The Art and Science of Communication in JACC: Basic to Translational Science:
Introduction Over the years, I have received many positive comments from colleagues about the exceptional quality of the visual abstracts that accompany the original research papers in JACC: Basic to Translational Science. Although visual or graphical abstracts have become an increasingly popular method of summarizing and communicating the key findings of scientific papers, particularly in journals focusing on biomedical sciences and life sciences, this was not always the case.
Recognizing Early Career Translational Investigators:
Introduction JACC: Basic to Translational Science is committed to supporting and promoting early career investigators who are pursuing translational science. To achieve this, the Journal will provide biographical information and a photograph of the first or senior authors who are early career investigators in the issue where their article appears. Additionally, this biographical information will be shared on our social media platforms at the time of publication.

Introducing the 2024 THT Shark Tank Edition of JACC: Basic to Translational Science:
Introduction The global socioeconomic burden imposed by heart failure (HF) continues to rise unabated. Despite significant advancements in medical and device therapies, the symptoms of HF generally worsen over time, and most patients ultimately will die secondary to the complications arising from HF.
Is it Time to Abandon the Use of P-Values in Early Phase Translational Trials: Why (Effect) Size Matters:
Introduction Phase 1 clinical trials are primarily designed to test the safety of new drugs or devices. However, increasingly, phase 1 studies are being used as hypothesis-generating studies to help inform endpoints for larger phase 2 studies. Traditionally, researchers have relied on assessing a number of different possible endpoints using P values as the primary way of determining the significance of their findings.
Recurrent Adrenergic Stress Provokes Persistent Myocarditis in PD-1–Deficient Mice:
Introduction The immune system is essential for coordinating host responses to environmental danger, most notably through the detection and elimination of intruding pathogens,1 as well as by recruiting immune cells to sites of tissue injury to initiate tissue repair responses that are essential for maintaining host homeostasis and fitness.2−4 The ability of the immune system to simultaneously protect against both invading pathogens and orchestrate tissue repair processes is the result of...
Recurrent Myocardial Injury Leads to Disease Tolerance in a Murine Model of Stress-Induced Cardiomyopathy:
JACC Journals › JACC: BTS › Archives › Vol. 8 No. 7 Highlights • A single intraperitoneal dose of ISO provoked an acute inflammatory response in the heart that was accompanied by reversible changes in LV structure and function, whereas a second injection of ISO 7 days later had no effect on cardiac myocyte cell death, inflammation, or changes in LV structure and function, demonstrating that prior ISO injury and inflammation confers a cytoprotective effect in the heart.

Artificial Intelligence Discusses the Role of Artificial Intelligence in Translational Medicine: A JACC: Basic to Translational Science Interview With ChatGPT
Introduction Like many people, I was completely taken in and simultaneously taken aback by the public release of ChatGPT (GPT stands for “generative pretrained transformer”), the natural language processing tool that allows one to have a personalized conversation with an artificial intelligence (AI) bot capable of providing detailed responses to questions (prompts) with uncanny speed.

Introduction to Board Game Videography
Now that board gaming has become more and more popular throughout the Western world, a wide number of videographers of the hobby have sprung up with their own websites or on YouTube. Many are amateurish, working without scripts and failing to give their viewers a clear view of the game being reviewed (either in terms of rules or camera focus).
In Defense of Board Games
There’s been a renaissance in board games over the last decade or so in North America. Just a few years ago this seemed incredibly unlikely; after all, Microsoft and Sony were developing ever more sophisticated video game consoles, people were spending longer and longer hours in front of their black mirrors, and by the second decade of the twentieth-first century social life became dependent on Mark Zuckerberg’s brainchild let loose on the nascent Net. Yet it happened.
Sci-Fi TV and the Fall of Utopian Technology
Star Trek and the Liberal Utopian Dream When Gene Roddenberry first pitched Star Trek to NBC, he framed it as an epic voyage of rugged space pioneers akin to the westerns that then dominated the airwaves. What we got over the next three years was a deep exploration of two separate but linked phenomena: modern liberal politics and utopian technologies. By the former, I don’t mean what now passes for liberalism.

Reappraising the role of inflammation in heart failure
Abstract The observation that heart failure with reduced ejection fraction is associated with elevated circulating levels of pro-inflammatory cytokines opened a new area of research that has revealed a potentially important role for the immune system in the pathogenesis of heart failure.
Heart Failure With Recovered Left Ventricular Ejection Fraction: JACC Scientific Expert Panel
Abstract Reverse left ventricular (LV) remodeling and recovery of LV function are associated with improved clinical outcomes in patients with heart failure with reduced ejection fraction. A growing body of evidence suggests that even among patients who experience a complete normalization of LV ejection fraction, a significant proportion will develop recurrent LV dysfunction accompanied by recurrent heart failure events.

Digital culture is fast, light — and lazy | The Star
Recently I started plowing through a bunch of YouTube prog-rock videos from the late ’60s and ’70s, amazed at the musicianship of the bands in live performances: here were Gary Brooker and B. J. Wilson of Procol Harum on piano and drums respectively playing “Pilgrim’s Progress,” there was Keith Emerson of The Nice rolling through his take on Bach’s Third Brandenburg Concerto on his Hammond in “Brandenburger.” Every note played was analog, on real physical instruments, with no digital fixes added.
Epidemiology, pathophysiology and clinical outcomes for heart failure patients with a mid-range ejection fraction
We identified 168 patients with an LVEF between 40–50% at enrollment into a HF registry, and determined whether LVEF was improved, worsened, or the same compared to a prior LVEF. Three subgroups of HFmrEF patients were identified: HFmrEF improved (prior LVEF 50%); HFmrEF unchanged (prior LVEF 40–50%). The majority of patients (73%) were HFmrEF improved, 17% were HFmrEF deteriorated, and 10% were HFmrEF unchanged.
The Heart of Translation
I tried a number of different careers before I settled on medicine – I initially wanted to study music and then English. And though medicine was never Plan A it appealed because it allowed me to combine a love of science knowledge and the personal aspects of interacting with people. During medical school, I actually hated cardiology.
Training the Next Generation of Translational Cardiovascular Investigators: Is the Pipeline Half Full or Half Empty?
Introduction One of the major goals of JACC: Basic to Translational Science is to provide an open forum for discussions by translational cardiovascular investigators in academia and industry; patients and families affected by heart disease; the National Heart, Blood, and Lung Institute (NHLBI); and the U.S. Food and Drug Administration (1).
Prevalence of lactic acidaemia in patients with advanced heart failure and depressed cardiac output
Heart failure (HF) has been defined classically as a condition in which the heart is unable to deliver sufficient oxygen to match the needs of the metabolizing tissues. Surprisingly, this definition has never been validated. The goal of this study was to determine the prevalence of elevated lactate levels in a cohort of patients with advanced heart failure.
Left Ventricular Ejection Fraction and Risk of Stroke and Cardiac Events in Heart Failure
Background and Purpose—In heart failure (HF), left ventricular ejection fraction (LVEF) is inversely associated with mortality and cardiovascular outcomes. Its relationship with stroke is controversial, as is the effect of antithrombotic treatment. We studied the relationship of LVEF with stroke and cardiovascular events in patients with HF and the effect of different antithrombotic treatments.
