Dear Editor, Prasanna et al. [1] report longer median survival measured from cohort entry in patients with rheumatoid arthritis-associated interstitial lung disease (RA-ILD) than in controls with ILD (6.75 vs. 3.83 years), and suggest earlier detection as one explanation. The difficulty is that the survival clock begins before ILD is established. For patients with RA, follow-up began at the first RA admission; for controls, at a matched hospital admission.
