Exploring a postoperative alternative to intraoperative therapeutic plasma exchange for sensitized lung transplant recipients
We read with interest the analysis from Saddoughi et al from the Toronto group on the association between intraoperative therapeutic plasma exchange (iTPE) and bleeding in patients undergoing lung transplant when crossing known donor-specific antibodies (DSA).Their group is well positioned to tackle this question, having published pioneering work in this area in 2015.The present report addresses a perennial concern about the use of iTPE among surgeons, specifically that, in addition to the...
Persistent methicillin-resistant Staphylococcus aureus bacteremia from an infected superior vena cava thrombus
Future CardiologyVol. 19, No. 2 Case Report Abstract Infection is among the leading causes of mortality for patients with end-stage renal disease. The placement of catheters for hemodialysis are common culprits of infection and have been associated with the development of complications such as venous thrombosis, bacteremia and thromboembolism.
Anomalous left anterior descending coronary artery arising from the right coronary artery: a case report
Future CardiologyAhead of Print Case Report Abstract Congenital anomalous coronary artery origins are rare, with a prevalence of 0.24–1.6%. Common configurations include left circumflex arising from the right coronary (RCA), left main coronary arising from the right coronary sinus and independent ostia for the left anterior descending (LAD) and left circumflex. One rare configuration that is not well described is the LAD arising from the RCA.
Clinical transplantation using negative pressure ventilation ex situ lung perfusion with extended criteria donor lungs
Abstract Lung transplantation remains the best treatment option for end-stage lung disease; however, is limited by a shortage of donor grafts. Ex situ lung perfusion, also known as ex vivo lung perfusion, has been shown to allow for the safe evaluation and reconditioning of extended criteria donor lungs, increasing donor utilization.
A Decrease in Hypoxic Pulmonary Vasoconstriction Correlates With Increased Inflammation During Extended Normothermic Ex Vivo Lung Perfusion
Normothermic ex vivo lung perfusion (EVLP) is an evolving technology to evaluate function of donor lungs to determine suitability for transplantation. We hypothesize that hypoxic pulmonary vasoconstriction (HPV) during EVLP will provide a more sensitive parameter of lung function to determine donor lung quality for lung transplantation. Eight porcine lungs were procured, and subsequently underwent EVLP with autologous blood and STEEN solution for 10 h.
