Hardware-operable failures (HOFs) interrupt large language model (LLM) training but permit recovery on the same hardware without reset, repair, or replacement. Existing recovery systems nevertheless reload checkpoints, recompute lost progress, and rebuild process state, idling GPUs that could otherwise continue training. We present Leto, a fault-tolerant training system that leverages surviving hardware to enable efficient in-place recovery.

Abstract Efficient and accurate methods to estimate insulin sensitivity (SI) and beta-cell function (BCF) are of great importance for studying the pathogenesis and treatment effectiveness of type 2 diabetes. Many methods exist, ranging in input data and technical requirements. Oral glucose tolerance tests (OGTTs) are preferred because they are simpler and more physiological.
Surgical site infection after hip fracture surgery
Article focus A large-scale meta-analysis of 20 studies from the UK considering over 80,000 patients. Analysis was undertaken according to major categories of surgery and study definition of infection. Key messages Rates of surgical site infection (SSI) after hip fracture in the published literature may be higher than surveillance suggests. Definitions used across studies were varied and may explain the variation in rates between studies.

Although it is well accepted that type 2 diabetes pathophysiology involves decreases in β-cell function and mass, their relative importance has been debated (1–3). Moreover, it is unclear whether β-cells adapt to insulin resistance by secreting more insulin per β-cell (increased function) or by increasing β-cell mass.A major reason why these questions remain unanswered is the lack of an animal model where dynamic changes in islet mass and islet function can be noninvasively monitored.
