Inflammatory biomarkers of asymptomatic and symptomatic tuberculosis
Abstract A large proportion of individuals with tuberculosis (TB) are asymptomatic. The biological and inflammatory underpinnings of asymptomatic TB are unknown and may differ from symptomatic TB. We characterise blood transcriptomic and proteomic profiles in South African community screening vs. health facility-based triage cohorts.
Inclusion of young adolescents in policy development for new tuberculosis vaccines
Introduction Mycobacterium tuberculosis is the single biggest cause of infectious disease mortality, with an estimated 1·23 million deaths and 10·7 million cases worldwide in 2024.1 The success of universal infant BCG vaccination for protection against childhood tuberculosis provides definitive evidence that tuberculosis can be prevented by vaccination.2–4 BCG vaccination of newborns offers protection against tuberculosis for approximately 10 years, with protection most evident when M...

Age and sex influence antibody profiles associated with tuberculosis progression - Nature Microbiology
Abstract Antibody features vary with tuberculosis (TB) disease state. Whether clinical variables, such as age or sex, influence associations between Mycobacterium tuberculosis-specific antibody responses and disease state is not well explored. Here we profiled Mycobacterium tuberculosis-specific antibody responses in 140 TB-exposed South African individuals from the Adolescent Cohort Study.
T cell receptor repertoires associated with control and disease progression following Mycobacterium tuberculosis infection - Nature Medicine
Abstract Antigen-specific, MHC-restricted αβ T cells are necessary for protective immunity against Mycobacterium tuberculosis, but the ability to broadly study these responses has been limited. In the present study, we used single-cell and bulk T cell receptor (TCR) sequencing and the GLIPH2 algorithm to analyze M. tuberculosis-specific sequences in two longitudinal cohorts, comprising 166 individuals with M.
Inflammation and immune activation are associated with risk of Mycobacterium tuberculosis infection in BCG-vaccinated infants - Nature Communications
Abstract Tuberculosis vaccine development is hindered by the lack of validated immune correlates of protection. Exploring immune correlates of risk of disease and/or infection in prospective samples can inform this field. We investigate whether previously identified immune correlates of risk of TB disease also associate with increased risk of M.tb infection in BCG-vaccinated South African infants, who became infected with M.tb during 2-3 years of follow-up.

Helen McShane and colleagues reply to Deborah Cohen
Cohen questions the justification for a large efficacy trial of the novel tuberculosis (TB) vaccine MVA85A in South African infants.12 This trial confirmed that MVA85A was safe and provided valuable understanding of the TB vaccine.3 These trial results were published within 10 working days of our first seeing the data.24 We strongly agree with calls for rigor in conducting, and openness in reporting, preclinical studies, but we are confident that moving forward with this efficacy trial was...
