Prison Focus Issue 51 California Institute for Women (CIW) is responsible for six suicides in the past two years, the highest rate of any California prison. Despite new leadership after the warden, Kimberly Hughes, forced retirement in August 2016, CIW has repeatedly failed to follow their own mental health crisis policies and procedures. To date, no correctional officers have lost their jobs despite the systematic, neglectful failures that led to these six deaths.
