Tired of making clinical decisions… Prompt and accurate decision-making is a necessity to deliver effective patient care. However, the impact of sleepiness during a nightshift may reduce cognitive functioning and clinical performance. This study sought to examine second-year paramedic students' clinical decision-making while they undertook a 13-hour simulated night shift concentrating on real-life clinical scenarios.

Reducing on-scene time using the PHTLS programme Recent studies suggest that in cases of serious trauma, reducing prehospital time could improve survivability of patients. An area of particular interest is the reduction in time spent on scene by emergency medical services (EMS) personnel. The Prehospital Trauma Life Support (PHTLS) programme is used and recognised worldwide, focusing on an approach to minimise on-scene times.

An essential series on clinical decision-making on scene
Back in 2004, the Emergency Medicine Journal (EMJ) created a series entitled ‘The ABC of community emergency care’, which was designed to update general practitioners who were experienced in the field and to serve as an introduction to those new to emergency clinical decision-making. The aim of the series was to describe the management of non-traumatic emergencies commonly encountered in community emergency care.

Journal of Paramedic Practice
Editor Comment In the September issue of the Journal of Paramedic Practice, we published a ‘comment’ (i.e. opinion) article by ambulance service communications directors on guidance for the use of social media, and their views on its value within the ambulance services.
Accuracy of ECG Chest Electrode Placements by Paramedics; an observational study
Abstract Background: The use of the 12-lead ECG is common in UK paramedic practice but its value depends upon accurate placement of the ECG-electrodes. Several studies have shown widespread variation in the placement of chest electrodes by other health professionals but no studies have addressed the accuracy of paramedics. The main objective of this study was to ascertain the accuracy of the chest lead placements by registered paramedics.
The peer review process and the constant search for reviewers
I have been the editor of the International Journal of Therapy and Rehabilitation for approximately three and a half years, and I am pleased with the amount of submissions that we receive, which is steadily increasing year on year. The review protocol for the journal is that every article has to go through a double-blind peer review process, and each article needs to have two reviews in order for a decision to be made.

Journal of Paramedic Practice
It was with real sadness and regret that I learnt of the untimely death of Professor Malcolm Woollard last month. Sadness that a friend and mentor had died far too early, and regret that the projects we had been developing will now have to continue without his presence, insight and guidance.
In the shadows of other health services
On 25 September, the Health and Care Professions Council (HCPC) launched its consultation on the qualification threshold entry level to the register for paramedics-a consultation period that runs until 15 December. Currently, the threshold is set at 'equivalent to Certificate of Higher Education' and is the lowest of any registered health profession in the UK. This is a defining moment for the paramedic profession, which has long existed in the shadows of other health vocations.
