#SELChat: A Black History Month Reading List
When I first started college, I attended a small Catholic university in downtown Pittsburgh. One of my favorite classes was Multiculturalism. The professor introduced me to a new way of thinking. She encouraged the class to question what we see on television, what we read in magazines, and what we hear on the news. Some of the most important questions to ask are, “From whose perspective is this story being told?
Ask the Treatment Expert ... Diabetic foot ulcers
Can you help us improve treatment of diabetic foot ulcers? Diabetic foot ulcers usually are located on weight-bearing plantar surfaces, the medial surface of the metatarsophalangeal joint, the lateral aspect of the fifth metatarsophalangeal joint, or the tips of the toes. The wounds have the following characteristics: • Tissue in the wound bed is usually pale pink or yellow. • Wound is usually located over a bony prominence. • Wound may or may not be surrounded by callus.
Ask the Care Expert about ... choosing dressings
With thousands of dressings on the market, which is the best for wound healing? There are many factors to consider when making a dressing selection. These include wound etiology, allergies, wound status, condition of the periwound area, cost of the dressing, and whether there is someone who can be taught to apply the dressing. An extremely important function of an optimal wound dressing is protecting the wound from additional trauma while providing thermoregulation.
Ask the Treatment Expert about ... nutrition assessments
How do nutrition assessment and interventions support pressure injury prevention? A nutrition assessment is not simply a weight taken weekly, biweekly, or monthly. Much more is involved, especially if the resident is at risk for developing a pressure injury or has a diagnosis of a wound.
Ask the Treatment Expert about ... pressure injury assessments
Why are formalized pressure injury risk assessments so important? Pressure injury risk assessments will assist with patient-centered care planning, improve care plans, provide information related to discharge planning, and, of course, address resident rights. Facility staff should take into account the mobility status of each resident on admission and at specified intervals thereafter, according to the National Pressure Ulcer Advisory Panel.
Ask the Treatment Expert about ... pressure injury prevention
On the heels of Pressure Injury Prevention Day (November 17), what are the most important measures for pressure injury prevention? Governmental and published research does show that there has been a small decline in pressure injury development over the past five years. But without a vigilant emphasis on prevention, long-term care facilities will continue to face the pressure injury dilemma. Pressure injury prevention programs help, the Centers for Disease Control and Prevention says.
Ask the Treatment Expert about ... diabetic foot ulcers
What is the role of our clinicians in treating foot ulcers associated with a diagnosis of diabetes? Almost 10% of Americans have a medical diagnosis of diabetes. Skilled nursing facility residents with diabetes require additional clinical assessments and interventions in order to prevent and treat its effects, including foot ulcers. It is helpful for clinicians to understand the etiology of foot injuries.
Ask the Treatment Expert about ... surgical incisions
I recently admitted my first resident with a surgical incision with staples intact in a hip wound. Do you have tips on how to care for this wound? First and foremost, ensure that you read and understand the surgeon's order. It must be incorporated into the resident's plan of care. The goals of care provided are wound closure and a return of the area to prior use and function. Be sure you clearly understand why the surgery was performed. An initial assessment is essential.
Ask the Treatment Expert about ... blisters
Are all blisters pressure ulcers and if they are, how should they be staged? All blisters are not pressure ulcers. A pressure ulcer is defined as a “localized injury to the skin and/or underlying tissue as a result of pressure, or pressure in combination with shear.” An initial resident assessment should include a pressure ulcer risk assessment and skin assessment. The risk assessment should provide information related to mobility, nutrition, moisture, sensory perception and general health status.
Ask the Treatment Expert about ... maggot therapy
I have a colleague who would like to trial maggot therapy. Can you provide some information about it? Larval (maggot) therapy is a form of biological debridement used to remove non-viable tissue in a wound. Historically, larvae were used for debridement between World War I until the 1940s, when the availability of antibiotics emerged. In the 1990s, larval therapy use reappeared because antibiotic-resistant bacteria increased rampantly.
