1 INTRODUCTION A key feature of MS is the persistent inflammatory activity in the CNS throughout the whole disease course. Disruption of the blood–brain barrier (BBB) and perivascular lymphocyte infiltration are associated with focal demyelination in early disease. In progressive forms, disease severity is related to a prevailing CNS-specific innate immune response, with persistent cellular infiltrates of the leptomeninges and perivascular spaces, and consequent accumulating tissue damage.
