Amyloidosis and 30-Day Outcomes Among Patients With Heart Failure: A Nationwide Readmissions Database Study
Abstract Background The burden of amyloidosis among hospitalized patients is increasing over time. However, amyloidosis remains an underdiagnosed cause of heart failure (HF) hospitalization among older adults. Objectives We investigated the prevalence and prognostic implications of amyloidosis among patients hospitalized with HF.
Prevalence and Prognostic Significance of Mitral Regurgitation in Acute Decompensated Heart Failure: The ARIC Study
Abstract Objectives This study investigates the prevalence and prognostic significance of mitral regurgitation (MR) in acute decompensated heart failure (ADHF) patients. Background Few studies characterize the burden of MR in heart failure. Methods The ARIC (Atherosclerosis Risk In Communities) study surveilled ADHF hospitalizations for residents ≥55 years of age in 4 U.S. communities. ADHF cases were stratified by left ventricular ejection fraction (LVEF): <50% and ≥50%.
Shifting Trends in Timing of Pacemaker Implantation After Transcatheter Aortic Valve Replacement
Introduction Transcatheter aortic valve replacement (TAVR) has grown exponentially since 2011 and is now preferred over surgical aortic valve replacement in most patients with severe aortic stenosis (1,2). Despite advancements in TAVR technology and increasing operator experience, injury to the conduction system during TAVR resulting in high-degree atrioventricular block (HAVB) necessitating permanent pacemaker (PPM) implantation remains an obstinate complication.
Trends in Costs and Risk Factors of 30-Day Readmissions for Transcatheter Aortic Valve Implantation
Highlights • Costs of index hospitalizations for TAVI in the U.S. have significantly declined from 2012 to 2017. • Costs associated with readmissions have declined from 2012 to 2017. • Heart failure, infection, and arrhythmia were the top 3 causes of readmission after TAVI in 2017. • Readmissions due to arrhythmia and their costs increased from 2012 to 2017.

Wake Forest University long‐term follow‐up of type 2 myocardial infarction: The Wake‐Up T2MI Registry
Abstract Background The Wake‐Up T2MI Registry is a retrospective cohort study investigating patients with type 2 myocardial infarction (T2MI), acute myocardial injury, and chronic myocardial injury. We aim to explore risk stratification strategies and investigate clinical characteristics, management, and short‐ and long‐term outcomes in this high‐risk, understudied population.
Premature Ticagrelor Discontinuation in Secondary Prevention of Atherosclerotic CVD
Ticagrelor is a cornerstone of modern antithrombotic therapy alongside aspirin in patients with acute coronary syndrome and after percutaneous coronary intervention. Adverse effects such as bleeding and dyspnea have been associated with premature ticagrelor discontinuation, which may limit any potential advantage of ticagrelor over clopidogrel. The randomized trials of ticagrelor captured adverse events, offering the opportunity to more precisely quantify these effects across studies.

Acute MI hospitalizations increased in young patients
The number of young adults who were hospitalized for acute MI increased from 1995 to 2014, and this trend was more distinct in young women, according to a study published in Circulation.
Elderly patients may fare better with TAVR vs. surgery
Patients aged 80 to 89 years with aortic stenosis may experience better outcomes after transcatheter aortic valve replacement, as compared with surgical AVR, according to new data. “We did this study in part because, in our clinical experience, we observed that patients above 80 years old tended to do better with TAVR over surgical AVR, regardless of their calculated surgical risk.
Acute MI hospitalizations in young adults rising, especially in women
Sameer Arora CHICAGO — Hospitalizations for acute MI in younger patients rose between 1995 and 2014, particularly in women, according to data presented at the American Heart Association Scientific Sessions. “Recent reports have shown an increasing prevalence of cardiovascular risk factors in young.
Inpatient complications from TAVR reduced over time
CHICAGO — In an analysis of transcatheter and surgical aortic valve replacement procedures, TAVR was associated with reduced rates of inpatient complications over time and had lower rates of in-hospital stroke, cardiogenic shock, acute kidney injury and blood transfusion than surgery. In 2015, the most recent year of the analysis, TAVR also had a lower rate of inpatient mortality compared with surgery.
Twenty Year Trends and Sex Differences in Young Adults Hospitalized With Acute Myocardial Infarction

Few studies have examined the incidence and risk factors of AMI among young patients, or whether clinical management differs by sex.
Twenty Year Trends and Sex Differences in Young Adults Hospitalized with Acute Myocardial Infarction: The ARIC Community Surveillance Study
Background: Sex differences are known to exist in the management of older patients presenting with acute myocardial infarction (AMI). Few studies have examined the incidence and risk factors of AMI among young patients, or whether clinical management differs by sex. Methods: The Atherosclerosis Risk in Communities (ARIC) Surveillance study conducts hospital surveillance of AMI in 4 US communities (MD, MN, MS, and NC). AMI was classified by physician review, using a validated algorithm.
Trends in Inpatient Complications After Transcatheter and Surgical Aortic Valve Replacement in the Transcatheter Aortic Valve Replacement Era
The results of the original PARTNER trial (Placement of Aortic Transcatheter Valve) led to the approval of transcatheter aortic valve replacement (TAVR) for patients with severe symptomatic aortic stenosis and prohibitive/high surgical risk.1,2 However, concerns were raised about high rates of complications, such as stroke, vascular complications, paravalvular leak, and the need for permanent pacemakers (PPM).
Racial disparities exist in management, outcomes of patients with non-STEMI
During a 15-year period, black patients with non-STEMI had more comorbidities and were less likely to receive guideline-based therapies than their white counterparts, according to a study published in the Journal of the American Heart Association.
Fifteen‐Year Trends in Management and Outcomes of Non-ST‐Segment-Elevation Myocardial Infarction Among Black and White Patients: The ARIC Community Surveillance Study, 2000-2014 | Journal of the American Heart Association
Clinical Perspective What Is New? In this community‐based surveillance of patients hospitalized with non–ST‐segment–elevation myocardial infarction over a 15‐year period, black patients were younger and had more medical comorbidities than white patients In comparison to white patients, black patients had less likelihood of receiving guideline‐based non–ST‐segment–elevation myocardial infarction therapies, including invasive angiography and revascularization.
Differences between type 2 vs. type 1 MI may affect non-STEMI metrics
Patients with type 2 MI are different from those with type 1 MI, and it may make sense to exclude them from non-STEMI quality metrics, according to a study published in the Journal of the American Heart Association. “Although there has been widespread acceptance of the concept of type 2 MI, the ICD coding system did not recognize type 2 MI as a separate entity until October 2017.
