Intra-articular hyaluronic acid (viscosupplementation) for osteoarthritis: is it effective?
SUMMARY Viscosupplementation with intra-articular hyaluronic acid derivatives and cross-linked polymers of hyaluronic acid is increasingly used to treat symptomatic osteoarthritis in the knee, hip and other joints. Most guidelines conditionally recommend against its use to treat knee osteoarthritis, and strongly or conditionally recommend against its use for other joints, indicating a large evidence-to-practice gap.
Plasma concentration monitoring of oral vancomycin
Letter In the comprehensive review on therapeutic drug monitoring of vancomycin by Legg et al,1 while not specifically stated, the article appears to refer to intravenous use of vancomycin. Vancomycin can also be used via oral or enteral route for the treatment of Clostridioides difficile-related pseudomembranous colitis. Of interest is the following comment in Therapeutic Guidelines: Antibiotic: ‘Systemic absorption of vancomycin can occur with oral or enteral administration.

Geographic variation in out‐of‐pocket costs for radiation oncology services
Med J Aust 2023; 219 (5): . || doi: 10.5694/mja2.52066 Published online: 4 September 2023 To the Editor: I refer to Liu et al's1 thought‐provoking analysis of out‐of‐pocket costs for radiation oncology services according to geography. Notably the dichotomous demarcation of location by Australian Standard Geographical Classification (ASGC; 1 = major city, 2 = other) significantly limits the impact of their intended message. The full article is accessible to AMA members and paid subscribers.

Covid-19 vaccination: the dangers of “values” imperialism
Shyan Goh, orthopaedic surgeon PO Box 13, Northmead, NSW 2152, Australia sgoh{at}hotmail.com In response to Schwartz on equitable global access to covid-19 vaccines,1 Lloyd-Sherlock and colleagues2 consider Indonesia’s approach to vaccination programmes as ageist. They attribute the prioritisation of working adults under 60 years over older people to age discrimination and injustice.
Only mad dogs and Englishmen go out in the mid-pandemic, Christmas or not.
Dear Editors It is the end of the year and the forthcoming traditional festivities starting in 2 days are on everyone's mind. The majority of the almost 60 million population are on the move, all in a hurry to buy gifts and food, while making their way to meet up with their family for the annual gathering on a massive scale.
We need to take back control of the definition of "herd immunity", but the medical fraternity first needs to get its own house in order
Dear Editors How and when did the term “herd immunity” become such a despised term, a concept embroiled in political controversy and ideological conflict? As reflected by an excellent article (ref 1) published this month, the introduction of this phrase into (human) medical terminology from veterinarian science in early 20th century is related to transmission mechanics of communicable diseases in epidemics and the role of immunization.
Covid-19: What’s in a name? Isolating the term “quarantine” from other contamination
Shyan Goh, orthopaedic surgeon PO Box 13, Northmead, NSW 2152, Australia sgoh{at}hotmail.com Although “quarantine” and “self-isolation” involve similar measures,1 their purposes are quite different.
Who will guard the guards? Covid-19 research may be incomplete, but experts are vital during this pandemic
Shyan Goh, orthopaedic surgeon PO Box 13, Northmead, NSW 2152, Australia sgoh{at}hotmail.com I share Armstrong’s concerns about research integrity and the peer review process.1 The inconsistencies surrounding hydroxychloroquine (HCQ) and its potential role in treating covid-19 is a case in point. Many covid-19 HCQ studies are published as preprints without peer review on websites like medRxiv.
The Grand Social Experiment at 70: Who is really paying for your NHS?
Whenever a reader points out that the UK (or Australia) spends less on healthcare as a percentage of gross domestic product than most countries of the Organisation for Economic Cooperation and Development (OECD),123 I simply ask: “Would you like to pay 60% more tax?” Although many European OECD countries are spending 25% more than the UK or Australia, in gross domestic product, they also tax individual wages 40-60% more.4 Some countries also expect separate co-payment contributions to their...
Generics have a chequered recent history
Shyan Goh, orthopaedic surgeonSydney, NSW 2152, Australiasgoh{at}hotmail.comI’m surprised that McCartney didn’t mention the US version of the phenytoin fiasco1; Martin Shkreli, the chief executive officer of Turing Pharmaceuticals was dubbed the most hated man in America in 2015 after a 5556% price hike of the antiparasitic drug Daraprim (pyrimethamine), from $13 (£10; €12) to $750 a tablet. He …
