This prospective, non-randomised, open-label cohort study with two active medication arms (semaglutide and metformin), was conducted between September 2021 (first patient, first visit) and March 2024 (last patient, last visit) at Mondriaan, a mental health care institution in the Netherlands. The study was approved by the Mondriaan Mental Health Care Institutional Review Board (IRB no. CWO21017) and conducted in accordance with the Declaration of Helsinki [28] and Good Clinical Practice.

This is a type 1 hybrid quasi-experimental study with a mixed-method matched design. This design allows for the assessment of the effectiveness of GOAL! on health-related outcomes while collecting and analyzing implementation data [41]. An intervention group (N = 50) will be compared with a control group (N = 50) who continue to receive care as usual and are matched by demographic characteristics.

Introduction 5-Methoxy-N,N-dimethyltryptamine (5-MeO-DMT) is a serotonergic psychedelic from the tryptamine class, that primarily acts as an agonist at the 5-HT1A and 5-HT2A receptors (1, 2), and that, when delivered via pulmonary inhalation, has a rapid onset (about 5–10 s) and a short duration (about 5–30 min) of psychoactive effects (2–5).
