Add-on sirolimus in refractory iMCD-TAFRO: a TAFRO-enriched 8-patient case series with paired single-cell profiling
Abstract Introduction: Anti–IL-6 therapy with tocilizumab (TCZ) is ineffective in a proportion of patients with idiopathic multicentric Castleman disease (iMCD), particularly the TAFRO subtype, and rescue options remain limited. Evidence for mTOR inhibition comes mainly from cohorts in which iMCD-TAFRO was underrepresented. Methods: Eight Japanese patients with refractory iMCD (iMCD-TAFRO, n=6; iMCD-NOS, n=1) or TAFRO syndrome without iMCD (n=1) received add-on sirolimus (1–2 mg/day).
Insights into the etiology of Castleman disease
Subjects: In this issue of Blood, Chan et al report on in-depth studies of identical twins with idiopathic multicentric Castleman disease (iMCD), which provides a unique opportunity to study a genetic background for this enigmatic disorder.1 Chan et al describe 2 germ line mutations that could play a role in the development of iMCD.
Three years of maintenance with VRD in multiple myeloma: results of total therapy IIIB with a 15-year follow-up
Multiple myeloma (MM) outcomes have continually improved over the past 2 decades.1 Overall survival (OS) is improved with lenalidomide maintenance therapy after hematopoietic stem cell transplantation (HSCT).2 Patients with high-risk MM generally experience more aggressive disease behavior and poorer outcomes than those with standard-risk MM, and the addition of a proteasome inhibitor (PI) in the maintenance phase is usually suggested for these patients to overcome these worse outcomes.

Ethnic Disparities in AL Amyloidosis Outcomes Among Hospitalized Patients in the United States - Clinical Hematology International
AL amyloidosis is a clonal plasma cell disorder that results from deposition of fragments of immunoglobulin light or heavy chain in tissues [1]. The diagnosis of AL amyloidosis is difficult and can be delayed, due to similarity in clinical presentation with other more common medical conditions. Health disparities are well-established in other plasma cell disorders including multiple myeloma [2].
The Impact of Morbidity and Disability on Attendance at Organized Breast Cancer-Screening Programs: A Systematic Review and Meta-Analysis
This systematic review found that comorbidity is associated with reduced odds of attendance, with the greatest odds of non-attendance for individuals with neurological (OR = 0.45) or psychiatric conditions (OR = 0.57) and disabilities (OR = 0.59), respectively. Furthermore, odds of attendance reduced with increasing multimorbidity, particularly when individuals have three or more conditions.
Plasma cells expression from smouldering myeloma to myeloma reveals the importance of the PRC2 complex, cell cycle progression, and the divergent evolutionary pathways within the different molecular subgroups | Leukemia
Abstract Sequencing studies have shed some light on the pathogenesis of progression from smouldering multiple myeloma (SMM) and symptomatic multiple myeloma (MM). Given the scarcity of smouldering samples, little data are available to determine which translational programmes are dysregulated and whether the mechanisms of progression are uniform across the main molecular subgroups.
How Ability, Motivation, and Opportunity Drive Individual Performance Behaviors in Projects: Tests of Competing Theories
Abstract Organizations continue to struggle with underperforming workforces in projects. The ability–motivation–opportunity (AMO) framework may hold the key to determining what factors can foster improved performance in projects. Despite decades of research, however, the precise interplay of ability, motivation, and opportunity, as well as how they influence behavior, remains largely unclear.
