Pharmacogenomics of anti-seizure medications in epilepsy: a systematic review of genetic predictors of response and tolerance

Despite 168 pharmacogenetic studies in epilepsy, only a small set of markers currently offers robust clinical utility. HLA-B*15:02, HLA-A*31:01 and CYP2C9 remain the only consistently actionable predictors of severe cutaneous reactions and phenytoin toxicity. Most evidence is concentrated in Asian and European cohorts and in older ASM, creating a mismatch with contemporary prescribing patterns and underrepresenting African and Admixed American populations.

ABSTRACT Background Effectiveness and tolerability of anti-seizure medications (ASM) vary widely among individuals with epilepsy. Genetic polymorphisms may influence pharmacokinetic and pharmacodynamic responses, yet available evidence is heterogeneous and inconsistently translated into clinical practice. Methods A systematic search of eight databases (2000–2024) identified observational studies assessing associations between genetic variants and ASM response or adverse drug reactions.
