Managing Hematological Toxicities Associated with Bispecifics in MM
Transcript: Saad Z. Usmani, MD, MBA, FACP: This is something that our colleagues in the community will need to learn over time. I think managing cytopenias in general in solid oncology patients is different. The threshold or the comfort of treating patients with a lower threshold of neutrophil counts or hemoglobin is…it’s easy for the [hematology] malignancy-treating physicians to consider going ahead with treatment and providing that kind of transfusion or growth factor support to patients.

Managing Hypogammaglobulinemia Associated with Bispecifics in MM
Transcript: Saad Z. Usmani, MD, MBA, FACP: Another issue that was brought up in your case was the hypogammaglobulinemia in these patients. This is something that we’ve started to observe when anti-CD38 [monoclonal antibodies] started to be incorporated into the myeloma therapies, even in the frontline or early relapse setting. Dr Tan, what’s the general IVIG [intravenous immunoglobulin] prophylaxis guideline that you typically institute for your patients?
Case 3: A 49-Year-Old with R/R MM Treated with Bispecifics and Develops Adenoviral Infection
Transcript: Saad Z. Usmani, MD, MBA, FACP: I’d like to move on to our third case. Dr Scordo, I believe you’re going to be discussing this particular patient. Michael Scordo, MD: Yes. M.S. is a 49-year-old woman with stage IIIA, IgG kappa, relapsed/refractory multiple myeloma [R/R MM], and she’s on a bispecific antibody. Her MM was diagnosed in 2012. Managing Toxicities in Patients With MM Treated with ASCT or Novel Therapies

We looked at dysgeusia in patients who are undergoing autologous stem cell transplant with high-dose melphalan, and what we were trying to do is measure taste disturbances specifically. So looking at the 5 different tastes and using drops of taste to assess taste changes over time after transplant.
Case 2: A 57-Year-Old With R/R MM Treated with Bispecifics and Develops Neuropathy

Managing Neurotoxicity Associated with Bispecifics in MM
Transcript: Saad Z. Usmani, MD, MBA, FACP: As we know, Dr Tan mentioned that we are seeing some delayed neurotoxicities of various kinds, including neurocognitive sensitive neuropathy. With the BCMA [B-cell maturation antigen]-directed treatments, I think spacing out the dosing schedule once patients get a response is keen on everyone’s mind.
Managing Dysgeusia Associated with Bispecific Antibody Treatment in MM
Transcript: Saad Z. Usmani, MD, MBA, FACP: In terms of the mechanism of action, it’s not well understood or studied, but that poses a therapeutic challenge. Is it what happens when we are traumatized and victimized, forced to disassociate, to live compartmentalized lives in order to survive?
