Introduction Diabetic nephropathy (DN) and glomerulonephritis are two distinct types of glomerular diseases that affect millions of people worldwide. A common hallmark of DN and glomerulonephritis is proteinuria. Various studies have shown that a key player in the development of proteinuria in glomerular diseases is the endothelial glycocalyx (Garsen et al., 2014; van der Vlag and Buijsers, 2020).

Summary Background Proteinuria is associated with many glomerular diseases and a risk factor for the progression to renal failure. We previously showed that heparanase (HPSE) is essential for the development of proteinuria, whereas peroxisome proliferator-activated receptor ɣ (PPARɣ) agonists can ameliorate proteinuria.
