Activity and Physiological Stress Within 90 Days After Minimally Invasive and Open Pancreatoduodenectomy : A Predefined Analysis of the DIPLOMA-2 Randomized Clinical Trial
Activity and Physiological Stress Within 90 Days After Minimally Invasive and Open Pancreatoduodenectomy: A Predefined Analysis of the DIPLOMA-2 Randomized Clinical Trial Key Points Question What are the differences in postoperative recovery after discharge—using activity tracking—of minimally invasive pancreatoduodenectomy (MIPD) vs open pancreatoduodenectomy (OPD) in patients with upfront resectable neoplasm in the pancreatic head or periampullary region?

Implementation of Best Practices in Pancreatic Cancer Care in the Netherlands
Key Points Question Can enhanced nationwide implementation of guideline-based best practices in the Netherlands in pancreatic cancer care improve 1-year survival? Findings In this Dutch multicenter, stepped-wedge randomized clinical trial including 5887 patients, 1-year survival was 24% after implementation of 5 best practices compared with 23% before, and did not significantly differ.
'Surgical outcome of adrenalectomy for stage IV lung cancer patients with mixed response to immunotherapy'
Background: Adrenal metastases are common in lung cancer patients. Adrenalectomy is advised in case of solitary metastasis. Nowadays, a new group of patients is referred for adrenalectomy: patients with advanced metastatic lung cancer with mixed response to immunotherapy. In these patients the primary lung cancer and most metastases showed good response to immunotherapy, while the adrenal metastases did not respond or were newly developed.
Standard versus prolonged antibiotic prophylaxis to reduce the rate of infectious outcomes after Whipple procedure (SPARROW): study protocol for a randomized controlled trial
Background: Pancreatoduodenectomy remains a complex procedure with high rates of morbidity (20-52%). Previous studies showed an association between prolonged antibiotic prophylaxis after pancreatoduodenectomy and a reduction of postoperative infectious complications, particularly in patients with preoperative drainage. However, most of these were observational studies and the only two, small randomized trials reported conflicting results regarding the rate of infectious complications.
Radical resection rates of pancreatic cancer in the era of centralization, neoadjuvant therapy, and standardized pathology reporting: a nationwide analysis
Background: Pancreatic cancer care continues to evolve due to centralization of care, (neo)adjuvant chemo(radio)therapy, minimally invasive surgery, and standardized pathology reporting. These changes may influence the rate of radical resection (R0). This study aims to give an overview of the effects of these changes on R0 rates for pancreatic cancer.
The yield of staging laparoscopy for resectable and borderline resectable pancreatic cancer in the PREOPANC randomized controlled trial
The necessity of the staging laparoscopy in patients with pancreatic cancer is still debated. The objective of this study was to assess the yield of staging laparoscopy for detecting occult metastases in patients with resectable or borderline resectable pancreatic cancer.
Bihormonal Artificial Pancreas With Closed-Loop Glucose Control vs Current Diabetes Care After Total Pancreatectomy: A Randomized Clinical Trial

JAMA Surg. 2022;157(10):950-957. doi:10.1001/jamasurg.2022.3702 Key Points Question What is the efficacy and safety of the novel bihormonal artificial pancreas (BIHAP) in patients after total pancreatectomy?
Prolonged antibiotic prophylaxis after pancreatoduodenectomy reduces abdominal infections
Daphne Droogh Coen van Dam Jesse Groen Bert Bonsing Alexander Vahrmeijer Bas Groot Koerkamp Sven Mieog Background: Abdominal complications account for approximately 40% of the complications following pancreatoduodenectomy. Positive bile cultures, predominantly occurring in patients undergoing preoperative biliary drainage, are related to the development of abdominal infections and infected bile spillage may account for this.

Circulating TP53 mutations are associated with early tumor progression and poor survival in pancreatic cancer patients treated with FOLFIRINOX
Introduction Although some advances have been made in the treatment of pancreatic ductal adenocarcinoma (PDAC), the prognosis of patients remains very poor.1,2 The standard first-line treatment for locally advanced pancreatic cancer (LAPC) and metastatic PDAC is FOLFIRINOX chemotherapy, a combination of fluorouracil, leucovorin, irinotecan, and oxaliplatin.
Nationwide trends in incidence, treatment and survival of pancreatic ductal adenocarcinoma
1. Introduction Pancreatic ductal adenocarcinoma (PDAC) is a devastating disease with a poor survival. Reported 5-year relative survival rates range around 8.5%, and in 2018 alone, 430,000 patients died from PDAC worldwide [1x[1]Global Cancer Observatory (GLOBOCAN). http://gco.iarc.fr/; 2018. Google ScholarSee all References1]. In the last two decades, studies showed an improved survival in patients with PDAC based on new oncological treatments.
Benefit of Surveillance for Pancreatic Cancer in High-Risk Individuals: Outcome of Long-Term Prospective Follow-Up Studies From Three European Expert Centers
|
Purpose Pancreatic ductal adenocarcinoma (PDAC) has a poor prognosis. Hereditary factors play a role in the development of PDAC in 3% to 5% of all patients. Surveillance of high-risk groups, may facilitate detection of PDAC at an early stage. The aim of this study was to assess whether surveillance aids detection of early-stage PDAC or precursor lesions (PRLs) and improves the prognosis.
