Digital intervention (Renewed) to support symptom management, wellbeing, and quality of life among cancer survivors in primary care: a randomised controlled trial
Abstract Background Many cancer survivors following primary treatment have prolonged poor quality of life. Aim To determine the effectiveness of a bespoke digital intervention to support cancer survivors. Design and setting This was a pragmatic parallel open randomised trial in UK general practices (ISRCTN:96374224).
Digital intervention (Renewed) to support symptom management, wellbeing, and quality of life among cancer survivors in primary care: a randomised controlled trial
cancer survivors health resources primary health care self-management Introduction Although the UK has one of the poorest cancer survival rates among higher-income settings,1 the prevalence of cancer survivors has been increasing year on year (https://www.cancerresearchuk.org) and by 2040 cancer survivors are likely to represent a quarter of the UK population.2 The quality of life (QoL) of some cancer survivors is poor, equivalent to chronic diseases3,4 and remains persistently poor over...

Internet and Telephone Support for Discontinuing Long-Term Antidepressants
Key Points Question Is adding internet and telephone support to a family practitioner review to consider discontinuing long-term antidepressants safe and more effective than practitioner review alone? Findings In this cluster randomized clinical trial including 330 enrolled adults, scores on the PHQ-9 depression questionnaire were slightly better in the supported arm after antidepressant discontinuation, as were scores for withdrawal symptoms and mental well-being.
The experiences of men on active surveillance for prostate cancer and their significant others: A qualitative synthesis
1 INTRODUCTION There is a dearth of literature exploring experiences of active surveillance (AS) for prostate cancer (PCa) qualitatively, and an even smaller pool of literature including experiences of significant others (SOs), despite it being frequently reported that partners are the primary source of support for men with PCa.1, 2 Prior to this review a psycho-educational support intervention for men on AS for PCa, named PROACTIVE,3 identified a gap in the evidence base regarding the role...
Physical activity and mental health in individuals with multimorbidity during COVID-19: an explanatory sequential mixed-method study
PA and functioning Despite restrictions, many participants developed coping strategies to enable them to deal with their condition, during the pandemic, depending on how the condition affected their lives and their ability to function at a given time. For many, PA was a central strategy to control symptoms, improve their mood and well-being, and slow the progression of the condition.

Using microbiological data to improve the use of antibiotics for respiratory tract infections: A protocol for an individual patient data meta-analysis
We will describe study and patient characteristics for all studies that contribute IPD, as well as those that declined or were unable to provide data. This will help to determine the extent to which the included studies are a representative sample of patients presenting in primary care with acute RTI. IPD meta-analysis can be conducted using either a two-stage or a one-stage approach.
Type 2 diabetes remission trajectories and variation in risk of diabetes complications: A population-based cohort study
PLOS ONE, Aug 2023 Biochemical remission of type 2 diabetes is achievable through dietary changes, physical activity and subsequent weight loss. We aim to identify distinct diabetes remission trajectories in a large population-based cohort over seven-years follow-up and to examine associations between remission trajectories and diabetes complications. Group-based trajectory modelling examined longitudinal patterns of HbA1c level (adjusting for remission status) over time.

Internet-Based Vestibular Rehabilitation for Older Adults With Chronic Dizziness: A Randomized Controlled Trial in Primary Care
Abstract PURPOSE Vestibular rehabilitation is an effective intervention for dizziness due to vestibular dysfunction, but is seldom provided. We aimed to determine the effectiveness of an Internet-based vestibular rehabilitation program for older adults experiencing dizziness in primary care.
Post-consultation acute respiratory tract infection recovery: a latent class-informed analysis of individual patient data
Implications for research and practice A similar antibiotic prescribing rate was observed across the different groups for cough, highlighting clinician uncertainty and a need for a better guide about which patients should be prescribed antibiotics. Many key characteristics identified here such as illness severity, prior duration of illness, and presence of lung disease can easily be assessed in primary care and can be used by clinicians to reassure patients and appropriately manage those at risk.

Margin status and survival outcomes after breast cancer conservation surgery: prospectively registered systematic review and meta-analysis
Research Margin status and...
Background: International guidelines state any post-surgical margin wider than tumour on ink (ToI) following breast conserving surgery (BCS) for invasive cancer is acceptable, based on analyses of margin width and local recurrence (LR). We aimed to determine if margin involvement is associated with distant recurrence (DR) and to determine a minimum margin to minimize DR and LR.
Effectiveness of steam inhalation and nasal irrigation for chronic or recurrent sinus symptoms in primary care: a pragmatic randomized controlled trial

Antibiotics for lower respiratory tract infection in children presenting in primary care in England (ARTIC PC): a double-blind, randomised, placebo-controlled trial
Summary Background Antibiotic resistance is a global public health threat. Antibiotics are very commonly prescribed for children presenting with uncomplicated lower respiratory tract infections (LRTIs), but there is little evidence from randomised controlled trials of the effectiveness of antibiotics, both overall or among key clinical subgroups.
Accuracy of Rapid Point-of-Care Antibody Test in patients with suspected or confirmed COVID-19
Highlights • The rapid test for antibodies to SARS-COV-2 was 90.1% sensitive and 100% specific • This is comparable to reference antibody tests for prior infection with COVID-19 • It could support clinical decision-making for patients with >10 days of symptoms • It would be useful in settings where vaccination is not widespread • And where PCR tests are not widely available Keywords COVID-19 rapid antibody test point-of-care lateral flow immunoassay sensitivity specificity Dear Editor, .
Re: Delayed antibiotic prescribing for respiratory tract infections: individual patient data meta-analysis - Authors' response to Dr Bradley
Dear Editor We thank Dr Bradley for his thoughts. As Dr Bradley suggests, there is no evidence that early treatment of suspected sepsis in primary care improves outcomes. Indeed, the key challenge in primary care is the timely identification of early sepsis, which can be indistinguishable from common infections early on. Observational data suggest that the risk of sepsis in RTI is lower than in other conditions and the number needed to treat is large.

A non-linear optimisation method to extract summary statistics from Kaplan-Meier survival plots using the published P value
Abstract Background Meta-analyses of studies evaluating survival (time-to-event) outcomes are a powerful technique to assess the strength of evidence for a given disease or treatment. However, these studies rely on the adequate reporting of summary statistics in the source articles to facilitate further analysis. Unfortunately, many studies, especially within the field of prognostic research do not report such statistics, making secondary analyses challenging.

I’m writing this in Skowhegan, Maine, at this residency that is, like, famous and stuff. There is a fresco shop here; not a dusty, unused remnant of another time, but a vital studio run with a mind to critical contemporary practice. On the second evening of the residency, in a cavernous barn lined with frescoes by American art heroes, Oscar Rene Cornejo gives a lecture about the medium. What is taught here is buon fresco. Probiotic capsules and xylitol chewing gum to manage symptoms of pharyngitis: a randomized controlled factorial trial
BACKGROUND: Reducing the use of antibiotics for upper respiratory tract infections is needed to limit the global threat of antibiotic resistance. We estimated the effectiveness of probiotics and xylitol for the management of pharyngitis. METHODS: In this parallel-group factorial randomized controlled trial, participants in primary care (aged 3 years or older) with pharyngitis underwent randomization by nurses who provided sequential intervention packs.
Predictors of pneumonia in lower respiratory tract infections: 3C prospective cough complication cohort study
AbstractThe aim was to aid diagnosis of pneumonia in those presenting with lower respiratory tract symptoms in routine primary care.A cohort of 28 883 adult patients with acute cough attributed to lower respiratory tract infections (LRTIs) was recruited from 5222 UK practices in 2009–13. Symptoms, signs and treatment were recorded at presentation and subsequent events followed-up for 30 days by chart review.
