We enrolled 2,962 subjects with sepsis between 2008 and 2022, of whom 371 (13%) had a pre-existing diagnosis of cirrhosis (Supplemental Fig. 2). In the overall cohort, 680 (23%) subjects developed ARDS, 1760 (59%) developed AKI, and 1312 (44%) died by 30-days. Confounder variables were missing in < 2% of subjects. Comparisons of patient characteristics by cirrhosis status are provided in Table 1. A higher percentage of patients with cirrhosis were male and were of younger age.

Recruitment flow is presented in Fig. 1. We identified 42 patients engaged in longitudinal follow-up for the parent cohort study and were approximately 1-year post-hospital discharge. We successfully contacted 27 patients, of whom 24 consented to participate. Thematic saturation was achieved after 24 interviews. Participant demographic and clinical characteristics are presented in Table 1 and Appendix C. The majority of participants (71%) identified as Black.

Abstract Purpose Although dozens of studies have associated vancomycin + piperacillin–tazobactam with increased acute kidney injury (AKI) risk, it is unclear whether the association represents true injury or a pseudotoxicity characterized by isolated effects on creatinine secretion. We tested this hypothesis by contrasting changes in creatinine concentration after antibiotic initiation with changes in cystatin C concentration, a kidney biomarker unaffected by tubular secretion.
