As a practicing doctor, medical academic, and occasional patient, I see everyday that the system is completely dependent on healthcare workers from every part of the world.

I grew up in the 1970s and remember watching rugby on terrestrial Irish television with groups of slightly overweight forwards manoeuvring around a patch of mud only to be interrupted by darting runs of skinny backs who managed more to evade than collide. I remember in 1995, the year the sport turned professional, first seeing Jonah Lomu playing in the backs, but built like a forward. His storming runs were a window on the future of contact sport.

Head trauma in sport has been linked previously to an increased risk of developing dementia, although why wasn't know. Back in 2023, we spoke with the former rugby union international Alix Popham who was diagnosed with early-onset dementia as a result, it's suggested, of repeated head concussions. Alix hung up his boots in 2011 owing to a shoulder injury, but his dementia symptoms developed later… Alix - Yeah, I was most probably in denial. My short-term memory was terrible.
