Abstract Primary sclerosing cholangitis (PSC) is an idiopathic, progressive and incurable liver disease. Here, we aimed for systematic analyses of adaptive immune responses in PSC. By profiling the T cell repertoires of 504 individuals with PSC and 904 healthy controls, we identified 1,008 clonotypes associated with PSC. A substantial fraction of these clonotypes was restricted to known PSC human leukocyte antigen susceptibility alleles and known to target Epstein–Barr virus (EBV) epitopes.

Abstract TCR diversity is essential for immune defense, yet the mechanisms underlying its decline with age, its dependence on sex and its variation among individuals remain poorly understood. These patterns are often attributed to passive loss from factors such as thymic atrophy and cumulative immune exposures but such processes fail to explain the systematic variation observed across populations.

Abstract ECOclusters (Exposure Co-Occurrence clusters) are previously described groups of public T-cell receptors (TCRs) that tend to co-occur across T-cell repertoires from tens of thousands of donors. Each ECOcluster putatively represents the public T-cell response to a different prevalent immune exposure. We previously associated a 26,106-member ECOcluster with exposure to cytomegalovirus (CMV) and used it to define a sensitive, specific classifier for CMV seropositivity.
