We read the article by Fathollahi et al. with great interest [1]. Fatholli and colleagues present a “zero infection protocol” for penile implant surgery which they claim can eradicate infectious complications [1]. Implant infection portends significant morbidity including penile shortening, corporal fibrosis, and an increased risk for complications in subsequent penile surgery [2, 3]. For this reason, infection prevention is of great interest to prosthetic urologists.

The study by Im et al. offers an important contribution to the ongoing discussion about infection prevention in penile prosthesis (PP) surgery, particularly in the high-risk context of revision procedures. By demonstrating that explanted hydrophilic PP surfaces can rebind vancomycin and gentamicin and subsequently reduce bacterial colonization ex vivo, the authors provide mechanistic evidence to support an intraoperative strategy that could have meaningful clinical implications [1].
