For years, point-of-care ultrasound (POCUS) training in internal medicine residency programs operated without a shared playbook. Individual programs built their own curricula, borrowed from emergency medicine frameworks, or offered little structured training at all. That changed in December when a consensus paper published in the American Journal of Medicine established the first expert- and evidence-based core curriculum for POCUS in internal medicine residency programs in the US.

And for just as long, many clinicians have worried about what they do to blood sugar. That concern has real biological grounding. Corticosteroids drive insulin resistance, and that reality has shaped counseling and monitoring protocols for years.

GLP-1 receptor agonists have reshaped the treatment of obesity and type 2 diabetes, but the prescription is only the starting point. Patients face a steep learning curve around nutrition, side-effect management, and long-term behavior change, and many turn to apps, wearables, and telehealth platforms to fill the gaps between office visits. Real-world persistence tells a tougher story.
