Lymphangiomas are rare benign tumors typically located in the head, neck, and axillae in children [1]. Cystic lymphangiomas are rare vascular connective malformative tumors resulting from sequestration of lymphatic tissue due to embryological developmental abnormalities in the lymphatic system [2]. In adults, they account for 7% of abdominal cysts [3]. Preoperative diagnosis is often challenging due to their frequently silent clinical course.

Case presentation We report the case of a 68-year-old Moroccan male with no significant medical history who presented with a perianal mass that had been gradually enlarging over a period of 6 months. Despite the chronic nature of the condition, the patient remained in good general health, although the mass caused discomfort during defecation and sitting. He consulted a visceral surgeon for evaluation.
