Ideas & Opinions - H. Michael Ross: Professional Engineers: First Do No Harm
I offer this personal note as an involved and informed professional civil engineer and as an NID rate and taxpayer in context to the professional fallout of the now abandoned Centennial Water Supply Project. The California Professional Engineers Act establishes that a professional engineer in responsible charge of a project must exercise “independent control and direction, by the use of initiative, skill, and independent judgment” (Section 6703) in their practice.
THE HEART OF INNOVATION: EVALUATING THE TRAFEROX COOLER'S IMPACT ON ISCHEMIC TIMES AND SHORT-TERM HEART TRANSPLANT OUTCOMES
Recent advancements in static controlled hypothermia technology have markedly enhanced donor heart preservation practices, particularly improving donor heart utilization and increasing ischemic times without compromising short-term outcomes. The Traferox X-Port Transport cooler (Traferox Technologies Inc., Toronto, ON) is a novel device that maintains donor hearts at a temperature range between 8 to 10⁰C.
THE RELATIONSHIP OF DIURETICS AND DIETARY SODIUM IN PATIENTS WITH HEART FAILURE: AN ANALYSIS OF THE SODIUM-HF TRIAL
SODIUM-HF was a large clinical trial testing dietary sodium restriction compared to usual care in patients with heart failure that showed no reduction in clinical events. It has been suggested that diuretic doses in response to dietary sodium modification may have influenced the trial results. Objective: We assessed the effects of baseline diuretic dose and diuretic dose changes on clinical outcomes in the SODIUM-HF trial.

SODIUM RESTRICTION IN PATIENTS WITH HEART FAILURE:A SYSTEMATIC REVIEW AND META-ANALYSIS OFRANDOMIZED CLINICAL TRIALS
Sodium restriction is a non-pharmacologic treatment suggested by guidelines for the management of patients with heart failure (HF). In this study, we synthesized the data from the randomized controlled trials (RCT) on the effects of sodium restriction on clinical outcomes in patients with HF. CARDIOVASCULAR CARE EXPERIENCES OF PEOPLE EXPERIENCING HOMELESSNESS AND THE FUTURE CO-DESIGN OF A CARE PROGRAM: A COMMUNITY-BASED STUDY IN TORONTO
Minimal consideration has been given to understanding the challenges of managing cardiovascular disease (CVD) and accessing mainstream cardiac care while homeless from the perspective of people with lived experience (PWLE). This community-based research aims to explore and understand the care experiences and needs of people experiencing homelessness and CVD to inform the co-design of a future care program in Toronto.
PREDICTORS OF MORTALITY IN PATIENTS TREATED WITH EXTRACORPOREAL MEMBRANE OXYGENATION FOR CARDIOGENIC SHOCK COMPLICATING ACUTE MI: A SYSTEMATIC REVIEW AND META-ANALYSES
Cardiogenic shock is associated with high mortality (as high as 50%) after acute myocardial infarction (MI). Extracorporeal membrane oxygenation (ECMO) has the potential to improve outcomes in these patients by providing hemodynamic support. However, the mortality rate for ECMO for acute MI is high which raises potential concerns about futility.
IMPACT OF REMOTE TITRATION COMBINED WITH TELEMONITORING ON THE OPTIMIZATION OF GUIDELINE DIRECTED MEDICAL THERAPY FOR PATIENTS WITH HEART FAILURE
Background To improve health outcomes in heart failure (HF) patients, guideline directed medical therapy (GDMT) should be optimized to target doses. However, GDMT remains underutilized, with less than 25% of patients receiving target doses in clinical practice. Telemonitoring is part of the management of HF and could provide reliable and real-time physiological data for clinical decision support to facilitate remote titration of HF medication.
VALIDATION OF THE INTERNATIONAL SOCIETY FOR HEART AND LUNG TRANSPLANTATION STANDARDIZED CARDIAC BIOPSY GRADING FOR ACUTE CELLULAR REJECTION IN HEART TRANSPLANTATION

Graft rejection post cardiac transplantation remains a significant cause of morbidity and mortality. The ISHLT reports that more than 20% of all heart transplant recipients experience rejection requiring therapy within one-year post-transplant. In this study, we evaluated the validity of the ISHLT Standardized Cardiac Biopsy Grading for Acute Cellular Rejection.

IMPACT OF HEALTH LITERACY ON KNOWLEDGE, SELF-CARE AND CLINICAL OUTCOMES IN HEART FAILURE PATIENTS
To view the full text, please login as a subscribed user or purchase a subscription. Click here to view the full text on ScienceDirect. The purpose of this study was to identify the prevalence of low health literacy (HL) and to determine whether low HL contributes to poorer disease knowledge, self-care, and clinical outcomes in ambulatory patients with heart failure (HF).
COMPARISON OF READMISSION AND DEATH AMONG CARDIAC PATIENTS IN NORTHERN VS. SOUTHERN ONTARIO

FiguresNorthern Ontarians (residents of the Northeast and Northwest Local Health Integration Networks) have higher population age-standardized mortality rates than the rest of the province, and the majority of deaths in Northern Ontario were attributed to circulatory diseases.
MANAGEMENT OF ACUTE DECOMPENSATED HEART FAILURE IN THE CORONARY INTENSIVE CARE UNIT: THE IMPORTANCE OF CO-MANAGEMENT WITH A DEDICATED HEART FAILURE SERVICE - Canadian Journal of Cardiology
To view the full text, please login as a subscribed user or purchase a subscription. Click here to view the full text on ScienceDirect. FiguresHeart failure (HF) affects more than 600, 000 Canadians and has been estimated to cost the health care system $2.8 billion annually. Patients admitted to a cardiac intensive care unit (CICU) incur a higher cost given the level of care provided.
PUT ME IN, COACH! : EXPLORING OPPORTUNITIES FOR NURSES TO ACT AS ADVANCE CARE PLANNING COACHES IN HEART FAILURE CARE
To view the full text, please login as a subscribed user or purchase a subscription. Click here to view the full text on ScienceDirect. Heart failure (HF) is a chronic, progressive and terminal illness with poor median survival and high mortality rates. The unpredictable and life-limiting nature of HF suggests that this population would benefit from advance care planning (ACP).
