Gastric adenocarcinoma is extremely rare in children, often manifesting with vague gastrointestinal symptoms. This case underscores the need to consider malignancy in pediatric celiac disease, particularly with poor dietary adherence. Awareness, early recognition, and multidisciplinary management are essential for improving outcomes in such rare, aggressive presentations.
