Background Immunoglobulin light chain (AL) amyloidosis is a rare but devastating monoclonal gammopathy where organ failure develops secondary to tissue deposition of misfolded immunoglobulin light chains. Aims We aimed to describe the characteristics and outcomes of a uniformly treated cohort of Australian patients with AL amyloidosis. Methods Subjects required a histological diagnosis of AL amyloidosis, symptomatic organ involvement and initial treatment with a bortezomib-based regimen.

Management The aim of treatment is to suppress the pathogenic monoclonal light chains as rapidly and deeply as possible. Attaining a very good partial response (VGPR; defined as a dFLC < 40 mg/L) or complete remission (CR) is the aim of therapy.36 Treatment within clinical trials is highly recommended in all newly diagnosed and relapsed/refractory patients. Treatment should be conducted in, or in collaboration with, a tertiary centre with extensive experience in the management of AL amyloidosis.
