Finerenone in patients with severe heart failure: The FINEARTS‐HF trial
Introduction Patients with heart failure (HF) with mildly reduced ejection fraction (HFmrEF) or preserved ejection fraction (HFpEF) experience an excess risk of morbidity and mortality worldwide, despite therapeutic advances.1, 2 A distinct subgroup with severe HF features is increasingly recognized, characterized by progressive symptoms, reduced tolerance to therapy, and frequent decompensations despite optimized medical therapy.3 Although severe HF was historically applied to individuals...

Effects of Sacubitril/Valsartan on All-Cause Hospitalizations in Heart Failure
Key Points Question What are the effects of sacubitril/valsartan on hospitalizations of any cause in patients with heart failure (HF)? Findings In this post hoc pooled analysis of the PARADIGM-HF and PARAGON-HF randomized clinical trials including 13 194 patients, sacubitril/valsartan significantly reduced the risk of all-cause hospitalization, primarily driven by lower rates of cardiac and pulmonary hospitalization.

Transcervical approach versus transfemoral approach for transcatheter aortic valve replacement
Highlights • Transcatheter aortic valve replacement via a transfemoral access is not possible in 15% of cases. • Transcervical and transfemoral accesses had similar results in terms of mortality, incidence of neurovascular complications, hospital length of stay, and improvement in aortic valve function. • Transcervical access may be a good alternative when transfemoral access is unavailable.
Use of SGLT2 inhibitors in cardiovascular diseases: why, when and how?

Pancytopenia in an adult patient with thiamine-responsive megaloblastic anaemia
Thiamine-responsive megaloblastic anaemia (TRMA) is a syndrome associated with megaloblastic anaemia, diabetes mellitus and sensorineural deafness, due to mutations in the SLC19A2 gene, which codes for a thiamine carrier protein. Oral thiamine supplementation is the main treatment. We report the case of a 25-year-old woman known for TRMA, who presented with pancytopenia (haemoglobin 7.6 g/dL, leucocytes 2.9×109/L, thrombocytes 6×109/L) revealed by dyspnoea.
