ABSTRACT BACKGROUND Per Centers for Disease Control and Prevention guidance, students with COVID-19 may end isolation after 5 days if symptoms are improving; some individuals may still be contagious. Rapid antigen testing identifies possibly infectious virus. We report on a test-to-return (TTR) program in a Massachusetts school district to inform policy decisions about return to school after COVID-19.

Key Points Question What is the rate of secondary transmission of SARS-CoV-2 in schools, and what factors are associated with transmission? Findings In this cohort study of 10 Massachusetts school districts, the secondary attack rate of SARS-CoV-2 in schools was 2.2% during the 2020-2021 school year and 2.8% in the fall of 2021. Factors associated with transmission in schools changed over time, although a greater social vulnerability index was associated with transmission in both periods.
