In this large observational study of approximately 45,000 voluntary, non-remunerated plasma donors, higher plasma donation rates were associated with lower observed infection risk. Importantly, this inverse association is likely due to bias inherent to observational studies, particularly the internal healthy donor effect, which limits interpretation and precludes causal inference. These findings highlight the challenges of evaluating the health effects of plasma donation using non-randomized data.
