Enrollment in health plans under the Affordable Care Act declined in nearly every North Carolina county this year, according to preliminary data released by the Centers for Medicare & Medicaid Services. Early 2026 data showed North Carolina had the sharpest drop nationwide — a 22% drop in the number of people enrolled in federal marketplace insurance. The expiration of enhanced subsidies at the federal level that pushed up monthly premium costs likely drove a sharp statewide enrollment decline.
