Obinutuzumab after anti-rituximab antibody-associated treatment failure in membranous nephropathy: a case report
Abstract Rituximab is an established therapy for primary membranous nephropathy, but anti−rituximab antibodies (ARA) have been associated with rituximab treatment failure. We report a 66−year−old woman with PLA2R−positive membranous nephropathy who developed ARA positivity and clinical features consistent with rituximab treatment failure, including B−cell reconstitution, undetectable serum rituximab levels, and worsening proteinuria despite declining anti−PLA2R antibody titers.
Early levothyroxine sodium administration and clinical outcomes in patients with sepsis: a MIMIC-IV database analysis
Abstract Sepsis is frequently complicated by non-thyroidal illness syndrome (NTIS). Although thyroid hormones are essential for cardiovascular and respiratory stability, their supplementation in sepsis remains controversial due to conflicting evidence. This study aimed to evaluate the impact of thyroid hormone supplementation on 28 day mortality, mechanical ventilation, vasopressor use, ICU length of stay, and changes in SOFA scores in patients with sepsis.

1 Introduction Patients admitted to the Intensive Care Unit (ICU) often present with life-threatening conditions and a high risk of mortality, highlighting the critical need for accurate outcome prediction tools. Among the most widely used scoring systems is the Acute Physiology and Chronic Health Evaluation II (APACHE II), which estimates mortality based on the most abnormal physiological parameters within the first 24 h of ICU admission (1).
