Cholangiocarcinoma 2026: status quo, unmet needs and priorities - Nature Reviews Gastroenterology & Hepatology
Abstract Cholangiocarcinoma (CCA) is a cancer that originates within the bile ducts. Traditionally considered to be a rare neoplasm, increased awareness of CCA alongside advancements in diagnosis and the rising prevalence of certain risk factors have contributed to a global increase in incidence and mortality. CCAs are highly heterogeneous from the clinical, histomorphological and molecular perspectives but commonly share a poor prognosis.

Personalising monitoring for chemotherapy patients through predicting deterioration in renal and hepatic function
1 INTRODUCTION Despite advances in targeted and immune therapies, cytotoxic chemotherapy remains the gold standard, first line treatment for many common cancers including breast cancer, colorectal cancer and diffuse large B-cell lymphoma (DLBCL).1 These anticancer agents are characterised by a narrow therapeutic index and large interindividual pharmacokinetic variability.
Integrated Clinical-Molecular Classification of Colorectal Liver Metastases
Key Points Question Are biologically derived molecular subtypes and integrated clinical-molecular risk stratification of colorectal liver metastases prognostic in an independent cohort from the phase 3 New EPOC randomized clinical trial? Findings Of 240 patients who underwent hepatic resection for limited colorectal liver metastases, the biological phenotype of each molecular subtype in the validation cohort was concordant with the discovery cohort.
Criteria for preclinical models of cholangiocarcinoma: scientific and medical relevance - Nature Reviews Gastroenterology & Hepatology
|
Abstract Cholangiocarcinoma (CCA) is a rare malignancy that develops at any point along the biliary tree. CCA has a poor prognosis, its clinical management remains challenging, and effective treatments are lacking. Therefore, preclinical research is of pivotal importance and necessary to acquire a deeper understanding of CCA and improve therapeutic outcomes.

Personalised drug therapy hope for bile duct cancer patients
Futibatinib works by targeting a particular genetic alteration, called FGFR2 fusion, which is found in around 14% of bile duct cancers. The researchers said their findings, published in the New England Journal of Medicine, offer hope to patients with this rare and aggressive cancer, which usually has a poor survival rate.
What is the gender representation in authorship in later phase systemic clinical trials in biliary tract cancer (BTC)? - a retrospective review of the published literature
Discussion Female first (1 in 5) and female senior (1 in 10) author representation in published later phase systemic clinical trial publications in BTC is low and has not changed significantly over the last two decades.

The co-evolution of the genome and epigenome in colorectal cancer - Nature
Abstract Colorectal malignancies are a leading cause of cancer-related death1 and have undergone extensive genomic study2,3. However, DNA mutations alone do not fully explain malignant transformation4,5,6,7. Here we investigate the co-evolution of the genome and epigenome of colorectal tumours at single-clone resolution using spatial multi-omic profiling of individual glands.
Long-Term Outcomes and Exploratory Analyses of the Randomized Phase III BILCAP Study
Article Tools ORIGINAL REPORTS Gastrointestinal Cancer Abstract Full Text PDF Figures and Tables Supplements Abstract The BILCAP study described a modest benefit for capecitabine as adjuvant therapy for curatively resected biliary tract cancer (BTC), and capecitabine has become the standard of care. We present the long-term data and novel exploratory subgroup analyses.
Reply to Comment on “The UK consensus position on the treatment of pancreatic cancer during the COVID-19 pandemic”
We read with interest Arshad et al.’s thoughts on our guidance, which was made available to and revised in response to feedback from clinicians from the outset of the COVID-19 pandemic.1,2 We note that the authors have not commented on any specific aspect of the article’s content; instead proffering their appraisal of the authors, coupled with a call for business as usual alongside an inference that amidst an unprecedented pivot in NHS care towards patients with COVID-19 at a time when up to...
Cholangiocarcinoma miscoding in hepatobiliary centres
Abstract Introduction Cholangiocarcinoma (CCA) are sub-divided into intrahepatic (iCCA) or extrahepatic (eCCA). eCCA are further subdivided into perihilar (pCCA) and distal (dCCA). Current and previous versions of the WHO International Coding of Disease and Oncology classifications (ICD) have separate topography codes for iCCA and eCCA, but none for pCCA. Over recent decades, multiple studies report rising incidence rates of iCCA with declining rates of eCCA, without reference to pCCA.
Current and novel therapeutic opportunities for systemic therapy in biliary cancer
Abstract Biliary tract cancers (BTCs) are a group of rare and aggressive malignancies that arise in the biliary tree within and outside the liver. Beyond surgical resection, which is beneficial for only a small proportion of patients, current strategies for treating patients with BTCs include chemotherapy, as a single agent or combination regimens, in the adjuvant and palliative setting.
Considerations for the treatment of pancreatic cancer during the COVID-19 pandemic: the UK consensus position
Abstract The coronavirus disease 2019 (COVID-19) pandemic epicentre has moved to the USA and Europe, where it is placing unprecedented demands on healthcare resources and staff availability. These service constraints, coupled with concerns relating to an increased incidence and severity of COVID-19 among patients with cancer, should lead to re-consideration of the risk–benefit balance for standard treatment pathways.
Cholangiocarcinoma 2020: the next horizon in mechanisms and management - Nature Reviews Gastroenterology & Hepatology
Abstract Cholangiocarcinoma (CCA) includes a cluster of highly heterogeneous biliary malignant tumours that can arise at any point of the biliary tree. Their incidence is increasing globally, currently accounting for ~15% of all primary liver cancers and ~3% of gastrointestinal malignancies.
Silver Linings: An Opportunity to Improve Clinical Paradigms After the COVID-19 Pandemic
The COVID-19 pandemic has greatly disrupted clinical workflows and standard management algorithms. Many hospitals have responded to infection risks and resource constraints accordingly by curbing nonurgent procedures and demarcating the number of intensive care unit (ICU) beds that patients without COVID-19 can occupy.1,2 As a result, clinicians have been tasked with devising alternative management approaches that comply with these limitations.

Systemic therapies for intrahepatic cholangiocarcinoma
Summary Intrahepatic cholangiocarcinoma (iCCA) is a highly lethal hepatobiliary neoplasm whose incidence is increasing. Largely neglected for decades as a rare malignancy and frequently misdiagnosed as carcinoma of unknown primary, considerable clinical and investigative attention has recently been focused on iCCA worldwide. The established standard of care includes first-line (gemcitabine and cisplatin), second-line (FOLFOX) and adjuvant (capecitabine) systemic chemotherapy.
Systemic chemotherapy with or without cetuximab in patients with resectable colorectal liver metastasis (New EPOC): long-term results of a multicentre, randomised, controlled, phase 3 trial
Summary Background The interim analysis of the multicentre New EPOC trial in patients with resectable colorectal liver metastasis showed a significant reduction in progression-free survival in patients allocated to cetuximab plus chemotherapy compared with those given chemotherapy alone. The focus of the present analysis was to assess the effect on overall survival. Methods New EPOC was a multicentre, open-label, randomised, controlled, phase 3 trial.
