Background: Neoadjuvant chemotherapy in combination with (chemo)radiotherapy has been suggested as potential new treatment strategy in patients with locally advanced rectal cancer. Nevertheless, discrepancies in patient selection and treatment regimens among studies hamper the clinical implementation. It has been hypothesised that patients facing the worst prognosis might benefit most from the addition of systemic treatment.
