Prescription drug affordability is a critical problem, with the KFF reporting that 4 of 10 individuals in the US face challenges in taking drugs as prescribed due to cost. Traditionally, policymakers have focused on mechanisms to control or administratively set pharmaceutical prices. An alternative approach is to enhance product competition, which is a more effective, durable solution that balances affordability and innovation.

Failing to Account for Real-World Complexities in Health Policy

The 2025 shutdown of the federal government—the longest in history—ended without a resolution to the impasse over extending the Affordable Care Act (ACA) enhanced subsidies. Beginning in 2021, the subsidies expanded assistance to families with incomes above 400% of the federal poverty level, reducing out-of-pocket costs for those enrolled in ACA health insurance. The subsidies expired at the end of 2025.
