Abstract Hip fractures cause major morbidity, mortality and long-term disability among older persons worldwide. The World Health Organization has defined two key indicators within the framework of the UN Decade of Healthy Ageing to measure health system performance in providing care for older adults with hip fractures: the proportion who receive surgery within 48 h of fracture; and the proportion who receive pharmacological treatment for osteoporosis post-fracture.

1 Introduction Osteoporosis is a major public challenge in the Asia–Pacific region where it affects ≤ 30% of women aged ≥ 40 years and ≤ 10% of men in developed economies [1]. Although many national guidelines within the Asia–Pacific region offer tailored strategies, differences in screening, risk assessment and treatment thresholds highlight the need for stronger, evidence-driven strategies to optimize patient outcomes and the importance of regional dialogue and knowledge exchange.
