Sir Nicholas Grimshaw: beyond the buildings and accolades
Mark Middleton pays tribute to Sir Nicholas Grimshaw, remembering a leader and mentor whose clarity, curiosity and belief in others defined his approach to architecture and practice The reflections on Nick’s life since his passing have rightly focused on his extraordinary contribution to architecture and the practice he founded. But he was always more than the sum of his buildings and awards. As a student, Grimshaw was the practice I most admired and the place I wanted to work.

Preferences for multi-cancer tests (MCTs) in primary care: discrete choice experiments of general practitioners and the general public in England - British Journal of Cancer
Abstract Multi-Cancer tests (MCTs) hold potential to detect cancer across multiple sites and some predict the origin of the cancer signal. Understanding stakeholder preferences for MCTs could help to develop appealing MCTs, encouraging their adoption. Discrete Choice Experiments (DCEs) conducted online in England. GPs (n = 251) and the general public (n = 1005) preferred MCTs that maximised negative predictive value, positive predictive value, and could test for a larger number of cancer sites.

CMV serostatus is associated with improved survival and delayed toxicity onset following anti-PD-1 checkpoint blockade - Nature Medicine
Abstract Cytomegalovirus (CMV) is a globally endemic latent herpes virus that profoundly impacts T cell immunity. We investigated the oncological consequences of CMV infection across 341 prospectively recruited patients receiving immune checkpoint blockade (ICB) for melanoma. CMV+ patients with metastatic melanoma (MM) have higher lymphocyte counts, reduced neutrophil to lymphocyte ratio and divergent CD8+ T cell gene expression.
Distinct immune cell infiltration patterns in pancreatic ductal adenocarcinoma (PDAC) exhibit divergent immune cell selection and immunosuppressive mechanisms | Nature Communications
|
Abstract Pancreatic ductal adenocarcinoma has a dismal prognosis. A comprehensive analysis of single-cell multi-omic data from matched tumour-infiltrated CD45+ cells and peripheral blood in 12 patients, and two published datasets, reveals a complex immune infiltrate. Patients have either a myeloid-enriched or adaptive-enriched tumour microenvironment. Adaptive immune cell-enriched is intrinsically linked with highly distinct B and T cell clonal selection, diversification, and differentiation.

Estimating Long-Term Survivorship Rates Among Patients With Resected Stage III/IV Melanoma: Analyses From CheckMate 238 and European Organization for Research and Treatment of Cancer 18071 Trials | Journal of Clinical Oncology
ORIGINAL REPORTS Introduction Melanoma accounts for 90% of all skin cancer-related deaths.1 The intention of adjuvant therapy in patients with melanoma is to prolong recurrence-free survival (RFS), that is, to reduce or eliminate the risk of recurrence or disease-related mortality and ultimately extend overall survival (OS).
Cyber-attacks on healthcare providers are a critical concern as they can directly impact patient safety. In many cases, IT system outages cause considerable disruption to patient care, as the lack of access to patient data results in delays that can affect patient outcomes. Multiple studies have identified increased hospital mortality rates following ransomware attacks and other major cyber incidents.
If you’re considering a move to Australia, expect ‘heaps’ of difference
New mutual recognition agreements (MRAs) for architectural qualifications are now a reality between the UK and the US, and as of the 25th of May, between the UK and both Australia and New Zealand. In early May I met Alan Kershaw, chair of the Architects Registration Board (ARB) in our Sydney studio and we discussed the significance of the agreement and then celebrated at an event hosted by the British Consulate.
Want to work in the US? The difference is more than just feet and inches
Since qualifying as an architect in 1992, my only relationship with the ARB has been paying my annual retention fee, leaving me wondering each year, Pythonesque ‘what have the ARB ever done for us?’ After 31 years, I couldn’t give a positive answer, but with the recent announcements regarding mutual recognition of architectural qualifications, which come into place on the 25 April for the US and the 25 May for Australia and New Zealand, the ARB are now roaring into my affections.
Opening an office overseas could be a smart move in the current climate
When I left the UK in late 2020 to “pandemigrate” to Sydney, we were grappling with a degustation of spiced Brexit aftermath, pandemic parfait, and a sour jus of uncertainty. You’d have thought things might have improved but like Monty Python’s Mr Creosote we are having our business cavities blown wide open by a wafer-thin mint of European war and whispers of global recession.

IL7 genetic variation and toxicity to immune checkpoint blockade in patients with melanoma - Nature Medicine
Abstract Treatment with immune checkpoint blockade (ICB) frequently triggers immune-related adverse events (irAEs), causing considerable morbidity. In 214 patients receiving ICB for melanoma, we observed increased severe irAE risk in minor allele carriers of rs16906115, intronic to IL7. We found that rs16906115 forms a B cell-specific expression quantitative trait locus (eQTL) to IL7 in patients.
A Phase 1B open-label study of gedatolisib (PF-05212384) in combination with other anti-tumour agents for patients with advanced solid tumours and triple-negative breast cancer | British Journal of Cancer
Abstract Background This Phase 1b study (B2151002) evaluated the PI3K/mTOR inhibitor gedatolisib (PF-05212384) in combination with other anti-tumour agents in advanced solid tumours. Methods Patients with various malignancies were administered gedatolisib (90‒310 mg intravenously every week [QW]) plus docetaxel (arm A) or cisplatin (arm B) (each 75 mg/m2 intravenously Q3W) or dacomitinib (30 or 45 mg/day orally).

Architects in recovery mode: Expect aggressive moves into new sectors
Grimshaw’s Mark Middleton on how practices need to overhaul their business plans in the pursuit of new work POSITIVE THINKING SERIES Does anybody know what is work and what is home anymore? Intratumoural immunotherapies for unresectable and metastatic melanoma: current status and future perspectives
Abstract The emergence of human intratumoural immunotherapy (HIT-IT) is a major step forward in the management of unresectable melanoma. The direct injection of treatments into melanoma lesions can cause cell lysis and induce a local immune response, and might be associated with a systemic immune response.

To Prevent the Next Pandemic, We Need Meaningful Oversight of Animal Agribusiness
We're all hoping that the current COVID-19 crisis will subside in a few months: that treatments, testing, and supplies will become available, and strict social distancing can safely ease. That may happen, but the threat of the next pandemic--an even deadlier one--is present now. We must recognize and address the root cause of this threat. We must reform the industry that is putting us all at risk in the name of profit: industrial animal agriculture.
Cash, clients, staff - and tough decisions
Mark Middleton has some practical advice for architects facing shrinking revenues Most architects who own businesses have little or no formal training to do the job, and for those who have made the leap and added an MBA I am sure they never had to pass a module on how to cope with a global pandemic. We are collectively in new territory, starting with zero experience of how to deal with the myriad challenges that we are faced with.
What you leave behind is not what is engraved in stone monuments. Discuss
The news of the death of Charles Jencks this month has sparked many reflections and stories about his influence on the world of architecture. I do not consider myself an expert in synaptic connections, so I have no idea why, but this sad news took my mind on a different journey. It jumped from Jencks to a BBC documentary from the 80s, via my old university in Dundee and a Peter Greenaway film and ended with a rumination on architects, architectural critics and their legacies.

No more excuses for green washing
We need to look for sustainable solutions at every stage of a project. But, says Mark Middleton, it’s not just the responsibility of architects World Green Building Week (WGBW; September 23-29) is an annual campaign to raise awareness of how the built environment can achieve a range of global goals such as mitigating climate change and creating both sustainable communities and economic growth.
What to expect from the new prime minister
We have a new prime minister. None of us voted for him and, barring a parliamentary coup, Boris Johnson will take the country out of the EU and into the next general election in May 2022. That’s just under three years living in Boris’s Britain. If we were to put all the politics, personality and no-deal angst to one side, what can architects expect from our new PM and his cabinet?

How (not) to win awards
By Mark Middleton2019-05-20T06:00:00 Too many practices fall into the same traps, says Mark Middleton who has been both a judge and a competitor over the years Awards are great, especially if you win one. The surprise and delight on the face of Jamie Fobert after receiving top honours at BD’s Architect of the Year Awards this month was testament to that.
The homeless crisis demands serious answers from architects
By Mark Middleton2019-04-04T06:00:00 Temporary shelters should be the preserve of student projects, says Mark Middleton Last year, I visited our new studio in Los Angeles and stayed downtown. On the first morning I took out my phone to order an Uber and on the screen it looked like I could walk there. With few opportunities to get anywhere on foot I thought I’d take this chance. As I left, the concierge asked if I needed a car so I told him I intended to walk.

It's time to put Nicholas Grimshaw in the spotlight
By Mark Middleton2019-02-14T07:00:00 As the architect receives the RIBA Royal Gold Medal tonight, Mark Middleton reflects on his mentor’s place in the canon As an architect, you rarely get time to look beyond the metronomic rhythm of projects and bids to reflect on the significance of your work. With that in mind, the start of this year has been different for everyone at Grimshaw.
Scrapping Tier 2 visas is a great step forward - but what about students?
By Mark Middleton2019-01-07T07:00:00 UK architecture relies on talented young part Is and IIs but the new immigration system will exclude them, warns Mark Middleton In early December, the RIBA and the Home office announced that a new bespoke category of visa for “exceptional talent” in architecture would be available from January 10, 2019. I for one greeted this with a yawn and a thought the architectural powers that be are answering the wrong question.

Long-term survival with anti-PD-1-based immunotherapy, but what is the best approach?
Checkpoint inhibitor immunotherapy has transformed the treatment of melanoma in the past 7 years, and can even lead to durable long-term survival outcomes in some patients with metastatic disease. However, we still have much to learn about how best to use these drugs and how to mitigate their side-effects. The optimal combination regimens, treatment duration, patient selection, and the point in the disease trajectory at which immunotherapy can be deployed to best effect remain open questions.
