Background Tobacco use is linked to multiple cancer types and a quarter of cancer deaths. Tobacco smoke contains numerous mutagenic chemicals such as polycyclic aromatic hydrocarbons (PAH), however, the mutagenicity of many chemicals in tobacco products remains underexplored.

In a recent issue of The Journal of Pathology, Huang et al [1] reported that chronic kidney disease (CKD) and end-stage renal disease (ESRD) are risk factors for hepatic angiosarcoma (HAS) in the Taiwanese population. A significant proportion of ESRD-associated HAS analysed in this study exhibited high tumour mutation burden with a predominance of A:T>T:A transversion-enriched COSMIC mutational signature SBS22 [1], which is indicative of exposure to aristolochic acid (AA) [2, 3].
